# Abstract

Depuis plus de 50 ans, les centres de santé communautaire (CSC) de l’Ontario font la prestation de soins de santé primaires complets selon les principes communs décrits dans le Modèle de santé et de bien-être. À l’instar d’une maison de quartier, nos organisations membres vont au-delà de leurs portes pour améliorer la vie des gens dans leurs communautés.

For over 50 years, community health centres (CHCs) in Ontario have delivered comprehensive primary health care according to the shared principles described in our Model of Health and Wellbeing (MHWB). Like a neighbourhood home, our member organizations reach beyond their doors to improve the lives of people in their communities. Although it began with CHCs, this approach is now embodied by organizations from multiple primary health care models, who together form the Alliance for Healthier Communities.

Community Health Centres (CHCs) in Ontario, Canada have been delivering health promotion programming as part of their model of comprehensive primary health care to communities for decades.
