# Welcome
The challenges faced by health care organizations, including fiscal restraint, a growing population, and difficulty recruiting and retaining staff, have led to a large number of people being unattached to primary health care and unable to access care when they need it. To address this crisis, primary health care teams across the province have been taking steps to increase their operational capacity, improving access and attachment to primary care for people in their communities.
The resources in this toolkit were collected by the Alliance for Healthier Communities to help organizations make the strategic and operational decisions necessary to implement and sustain these changes. These tools are intended to supplement tools provided by Ontario Health and others. Some were created by Alliance member organizations and generously shared with the Alliance to help others, and some were created by the Alliance with our members' help. Others are adaptations of older tools created by other organizations. We are grateful to our members and system partners for their willingness to share their resources, experiences, and learnings.

Complementary Toolkit from Health Commons Solutions Lab
Since the publication of this toolkit, another toolkit for primary care expansion was published by The Health Commons Solutions Lab, which can be used as a complementary resource. It features tools and case studies organized around the following themes: Expanding Access and Attachment to Primary Care, Creating your Implementation Plan, Outreach and Attachment, and Supporting Patients and Providers with Navigation. It also features a selection of case examples of different access & attachment models. A number of these are from Alliance member organizations.
