The successful candidate will work within a collaborative and interdisciplinary model of health and wellbeing, provide care and take part in planning and evaluation. Within the NP scope of practice, the successful candidate will provide primary care services in our College and Danforth Clinic. This role is vital in addressing population and individual health concerns, ensuring clients receive timely, high-quality care in a dynamic clinical environment. Through the Primary Care Clinic, the NP will support other primary care initiatives and work with specific immigrant, refugee and non-status populations that include women, 2SLGBTQI+, youth, children and families.

# About the position 

 Responsibilities: 

  • Providing episodic and primary care to clients of all ages within the scope of a RN-EC including assessments, diagnosis, screening, referral, education, treatment, counselling and follow up;
  • Delivering client-centered care for acute health issues, focusing on immediate needs and short-term interventions and prioritization of clients based upon complexity criteria for appropriate referrals;
  • Ability to manage an independent caseload that can include triage and urgent care; determines the need for orders and interprets screening and diagnostic laboratory tests, chest and limb x-rays, diagnostic ultrasounds and screening mammography;
  • Reviews client health, and, in cooperation with the client and other members of the team, direction for prioritization and recommendation concerning treatment, prevention and health promotion options; 
  • Tailor care approaches to meet the specific health requirements of the populations served, recognizing diverse health disparities;
  • Provides appropriate, treatment and supports continuity of care through maintaining complete and accurate client records; 
  • Participating in primary health care program and service delivery within the broader community context in partnerships with other institutions as may be required from time to time;
  • Participating in all organizational systems and structures as required including but not limited to, quality assurance and performance evaluation.

# Qualifications

 The ideal candidate will have a firm understanding of and a commitment to the principles and underlying values of community health centers/model of health and well-being as well as:

Required:

  • Masters prepared RN (EC) mandatory
  • Active registration and good standing with the College of Nurses of Ontario as a Primary Health Care Nurse Practitioner
  • Two years experience working as a primary care NP

Preferred:

  • Experience working with Newcomer populations
  • Experience in community or primary health care settings
  • Experience in an episodic and primary care or urgent care clinic or equivalent
  • Experience in conducting thorough health assessments that include gathering information about relevant medical/family history, pre-existing conditions, current medications
  • Strong cultural competency skills and ability to recognize and respect cultural differences
  • Experience with chronic disease management such as diabetes or hypertension and ability to provide education and support to patients and their families
  • Strong interpersonal skills, including active listening, empathy and effective communication
  • Thorough familiarity with therapeutic methods and practices based upon a health promotion/disease prevention model
  • Ability to work collaboratively within the interdisciplinary health care team which includes physicians, nurses, social workers, dieticians, physiotherapist, health coaches and community workers

Asset:

  • Knowledge of immigration policies and laws that affect the population served is preferred
  • Experience with trauma-informed care
  • Experience in navigation to help clients access the resources they need to receive quality care
  • Strong interpersonal communication skills (written and verbal) and problem-solving, organizational and time management skills
  • Strong digital/computer literacy skills and comfort with digital/virtual platforms and service delivery
  • Knowledge of issues affecting low income, multi-lingual, and racialized, and 2SLGBTQI+ communities
  • Experience with working with low-income, multilingual, multiracial populations and communities
  • Ability to speak multiple/second language(s)

When integrated into audit and feedback approaches, these measures have the potential to support more patient-centred, data-informed decision-making.

In this interactive, 60-minute webinar, Dr. Noah Ivers will open with relevant findings from the literature on Audit and Feedback. Dr. Sabrina Wong and Dr. Nathaniel Hawkins will then share perspectives and experience related to the use of PREMs/PROMs within audit and feedback, including considerations for advancing equity in practice and policy contexts. # About the position 

Summary:

The Nurse Practitioner is responsible for the provision of primary health care services in. accordance with the Standards of Practice for Registered Nurses in the Expanded Class and the Expanded Services for Patients Act, and other clinical activities as set forth by the College of Nurses of Ontario. The NP is responsible for ongoing health care, has a strong emphasis on health promotion, disease prevention and community capacity building, and works within a harm reduction and trauma-informed framework 

The ideal candidate will maximize connections and use of existing community resources by acting as a catalyst who, together with community stakeholders, will facilitate the development and implementation of creative and efficient strategies to maximize health outcomes for our clients, while maximizing the use of community care plan.

Key Objectives:

  1. Provide primary health care services within the community health centre, in community locations, satellite offices, home and hospital, and within the scope of the RN-EC professional standards including assessment, treatment, diagnosis, interpretation of test results, promotion of health education and counseling. history taking. screening, referral and follow up. This includes consultation with medical staff and other health care providers when the client's condition requires care beyond the scope of practice (RN- EC). 
  2. Ensure appropriate, comprehensive treatment is provided by maintaining complete and accurate charts and medical records and participate in chart reviews and case conferences.
  3. Working.in an interdisciplinary team to provide service delivery to support crisis stabilization on a short-term basis, including health care, housing, food, and other basic needs.
  4. Provides comprehensive, client-centred gender-affirming care across the lifespan within a primary care setting, aligned with World Professional Association for Transgender Health  General Services & Gender Affirming Care (WPATH) Standards of Care and Ontario-based clinical guidance, including referrals for gender affirming procedures.
  5. Supports clients with pre- and post-operative care within the CHC, including follow-up, complication management, and coordination with external surgical teams. 
  6. Supports clients in navigating complex healthcare, social service, and legal systems (e.g,name/gender marker changes, insurance, housing, income supports).
  7.  Advocates for equitable access to timely gender-affirming services at local, regional, and provincial levels. 
  8. Builds relationships with external partners (e.g., surgical programs, endocrinology, community organizations) to strengthen referral pathways. 
  9. Integrates gender-affirming care within broader primary care, including sexual health, chronic disease management, cancer screening, and preventive care tailored to individual anatomy and risk.
  10. Complete intake and coordinate treatment plans.
  11. Oversee activities required to ensure the clinic is fully equipped, adequately supplied and meets infection control standards and that controlled substances are monitored and accounted for.
  12. Supervise student/learners by providing competent leadership, orientation, developing and scheduling of training/learning plans including shared client contact.
  13. Act as part of an interdisciplinary team which assists with the development, analysis and implementation of policies and procedures for the benefit of improvement to the delivery of primary health care services.
  14. Participate in the Integrated Community Lead (ICL) Model, which includes participating in community meetings, organizing and hosting, and follow up for the ICL Model for complex care needs clients (e.g. Health Links clients). 
  15. Participate in and share responsibility for the on-call coverage offered by Quest CHC. 
  16. Provide input for development of strategic plans and attend and participate in staff meetings and special projects as assigned.
  17. Participates in CHC quality improvement initiatives, focusing on access, client experience, and health equity outcomes for trans and gender diverse populations. 
  18. Conduct presentations, facilitate workshops and group meetings as well as participate in program planning and community initiatives.
  19. Participate in and comply with all health and safety processes and initiatives to ensure the organization remains a safe workplace. 
  20. Promote the mission, vision, values and philosophy of care developed by Quest CHC.
  21. Develop and maintain strong ties with other professionals and community members.

 Education 

• A Master’s degree in Nursing required, or a combination of education and experience as sanctioned by the College of Nurses of Ontario.

Experience 

• Experience supporting individuals without a healthcare provider who are also experiencing mental health, addictions or concurrent disorders, street-involved populations (homeless, under-housed. sex trade workers), isolated seniors, sexually and gender diverse populations (2SLGBTQ+), at-risk children, youth and families. 

• Experience in sexual and reproductive health care inclusive of trans and non-binary clients.

Reporting to the Clinical and Allied Health Site Manager, Unison is seeking to fill a Medical Administrative Summer position at our Keele-Rogers site.

Job Responsibilities:

  • Provide secretarial and administrative support to the Clinical and Allied Health Site Manager
  • Support the clinical team administratively; take minutes as required
  • Maintain an efficient and organized office area, including orderly filing systems
  • Provide support for meeting logistics, including booking space and equipment, and ordering refreshments
  • Coordinate the ordering and tracking of office/meeting supplies for the teams
  • Maintain internal and external program contact lists and facilitate communications when required

# About the position 

  •  Clinical Services – performs assessments, diagnosis, treatments, health education/counselling and other medical functions based on appropriate standards of practice on a roster of primary care clients

  • Continuity of Care – conducts same-day access appointments, home visits as required and participate in the Centre’s on-call services on rotation to provide after-hours services as directed
  • Equity & Client-Centred Approach – works with clients, and engages with other services, in a manner that is mindful of the whole person/community within a social context and impacted by social determinants of health  
  • Quality Improvement – participates in continuous quality improvement 
  • Teamwork – participate in case conferences, makes referrals to interdisciplinary team and external resources, corresponding with a collaborative team and external resources
  • Administrative – contributes to the development, implementation and evaluation of clinical protocols and medical directives and other process improvements  
  • Mentoring – supports and evaluates students, medical residents, and other learners

# Qualifications

  •  RN (EC) license to practice from College of Nurses of Ontario 

  • Experienced as a Nurse Practitioner, with preference to those with experience in primary health care or in a community setting  
  • Experience working effectively with communities and individuals who face barriers accessing health care and other supports; with a focus on mental health challenges and problematic substance use preferred 
  • Demonstrated problem-solving, organizational, and interpersonal skills with an ability to work effectively and respectfully in a collaborative, interdisciplinary team environment  
  • Understanding of and experience utilizing a trauma-informed approach preferred

 The Opportunity 

As part of an inter-disciplinary team BQWCHC offers opportunities for physicians who are seeking a challenging, rewarding position providing comprehensive primary care services to clients at all ages and stages of the life cycle with an emphasis on those who are high risk and/or are experiencing barriers to accessing services. Our health equity focus brings together population health and the social determinants of health within an interdisciplinary primary health care model. 

BQWCHC offers a salaried model for compensation. Distinct from a fee-for-service or capitation model, a salaried model mitigates the challenges of dealing with a shrinking schedule of benefits, managing overhead costs and salaries. Benefits include bi-weekly pay (with deductions made at the source). As a salaried model, physicians are not required to do billing, manage staff or maintain responsibility for overhead.  

Physicians work as members of a team that includes nurse practitioners, nurses, social workers, pharmacist, dentist, dietician, Certified Diabetes Educator/CDPM nurse, housing worker, community resource worker and a host of other programs, groups and services to clients of BQWCHC and community members.  

Scope of Work 

  • Comprehensive primary health care  

  • Engagement in organizational initiatives on quality improvement, policy development, health equity issues and the development or modification of programs/service in respond to the needs of the community

  • CCFP (active status) 
  • General license to practice from CPSO in good standing. 
  • Current CMPA membership 
  • Primary care experience preferred, specifically with communities/individuals who face barriers to accessing care. 
  • Commitment to working within and optimizing the interdisciplinary team.
