 Planned Parenthood Toronto (PPT) is a fully accredited community health centre that provides primary health care services to youth 13 to 29 years as well as health promotion programming, education, training and research to improve the health and well-being of Toronto’s diverse communities. PPT offers health services at our downtown main site, our North York Satellite site, as well as through mobile health units that work across Toronto.

# About the position 

 We are currently seeking 2 Nurse Practitioners (RN(EC)). Shifts will  primarily be available during our Saturday clinics (shifts run from 9:30am to 2:30pm). Additional shifts will be available to cover vacation and other planned absences. Successful candidates must be available to work at least 1 Sat/month.

At PPT, Nurse Practitioners work within an interdisciplinary health care team to independently and collaboratively provide comprehensive primary and episodic health care to clients with an emphasis on sexual & reproductive health, health promotion, disease prevention and health education. Our Saturday Clinic appointments focus on sexual and reproductive health care needs but may include other primary care issues.

POSITION OVERVIEW                                                                                                                 

The Primary Care Nurse Practitioner Registered Nurse – Extended Class (RN(EC)) is a nurse specialist in primary health care with advanced knowledge and decision-making skills, who practices autonomously to offer the full scope of primary health care nurse practitioner practice.  The RN(EC) is expected to meet the standards of practice specific to the Extended Class and function in accordance with relevant legislation and standards of nursing practice set out by the College of Nurses of Ontario.

At PPT, the RN(EC) works within an interdisciplinary health care team to independently and collaboratively provide comprehensive primary and episodic health care to clients with an emphasis on sexual and reproductive health, health promotion, disease prevention and health education. The RN(EC) is responsible for supporting the Agency's mission, values and objectives. PPT is also committed to support NP learning - this position is an excellent opportunity to build your skills related to IUD and Implant insertions/removals, Medical Abortion, and Gender Affirming Care.

RESPONSIBILITIES

The Nurse Practitioner RN(EC), Locum/Casual practices within the scope of the Extended Class registration through the College of Nurses of Ontario and the policies and procedures of PPT. Physicians support and act as a primary liaison with the RN(EC) as needed in providing care to clients.

Direct client care:

  • Provides virtual, over-the-phone and in-person client-centred comprehensive primary and episodic health care to clients as outlined and appropriate within the Extended Class scope of practice.

  • Works independently and collaboratively with members of the interdisciplinary team; consults with other health care professionals when client’s condition is beyond the RN(EC) scope of practice and transfers care if appropriate. 

  • Provides health education, health promotion, and supportive counselling and/or crisis intervention to clients where appropriate.

  • Advocates on an individual basis for and with clients to other agencies and institutions.

  • Completes follow-up required by client and/or refers when appropriate.

  • Provides anonymous Standard HIV and Point of Care HIV testing with appropriate pre- and post-test counselling.

  • Provides educational information on IUDs and does IUD placements and removals with pain management options (training can be provided).

  • Provides educational information on the birth control implant and does implant insertions and removals (training can be provided).

  • Provides gender-affirming care and supports trans clients for hormone therapy and puberty suppression (training can be provided). 

  • Provides medical abortion medication to clients.

  • Supports clients with mental health concerns.

Administrative Tasks:

  • Maintains complete and accurate records of client visits including data required by Ontario Health.

  • Provides support in the development of Health Services protocols and collects data and other appropriate administrative tasks as required. 

# Qualifications

  • BScN or equivalent and membership in good standing with the College of Nurses of Ontario as an RN with Extended Class.

  • At least 3 years’ experience working as a Nurse Practitioner in an expanded role in a community-based setting.

  • Proficiency in current nursing physical assessment and treatment methods.

  • Experience and knowledge working with youth and sexual health issues.

  • Proficiency and experience working in a computerized environment.

  • Experience with Electronic Medical Records software an asset.

  • Excellent written and verbal communication skills.

  • Demonstrated commitment to equity goals and principles, sexual and reproductive rights and to being youth positive, 2SLGBTQ positive, sex-positive, sex work positive and to being inclusive of diverse sexual and gender identities.

 WellFort is seeking a Personal Support Worker to join our Homeless Health Program (HHP).

 

HOMELESS HEALTH PROGRAM

The Homeless Health Program is a Nurse Practitioner-led initiative that provides essential healthcare services to individuals who are homeless, precariously housed, or transitioning out of homelessness in the Peel Region. We understand the unique challenges faced by those without stable housing, and our goal is to make healthcare accessible, integrated, and responsive.

Our dedicated team includes Nurse Practitioners, Registered Nurses, Registered Practical Nurses, and Personal Support Workers, all working together to ensure our patients receive the care they need, where and when they need it. We remove barriers to care for those facing housing instability and poor health.

The program manages 45 medical beds at the Dundas Street Shelter, providing 24/7 care, as well as daytime mobile primary care services at other shelters and drop-ins across the region.

By providing these services, we help hospitals make the best use of their emergency and acute care resources. Patients who no longer need to stay in the hospital but still require medical support can continue their recovery in a shelter bed with healthcare services. We also offer additional support for people in shelters who may need extra medical attention, helping to prevent unnecessary hospital visits.

Our medical services include:

  • Care for chronic conditions like diabetes
  • Medication management
  • Support for substance withdrawal and pain management
  • Wound care
  • Mental health services and stabilization
  • Chiropody (foot care)
  • Monitoring and support after hospital stays

This program ensures that structurally vulnerable populations in our community have access to the care they deserve, improving overall health and wellbeing.

RESPONSIBILITIES:

The Personal Support Worker will be a valued and trusted companion to people experiencing homelessness by providing assistance with day-to-day living activities including personal care, meal set up, lifestyle assistance and light housekeeping duties. You will also encourage and assist the client in engaging in exercise, leisure and social activities to promote physical and mental wellbeing while fostering independence. The Personal Support Worker will work with their team to develop care plans, engage in constant client assessment, communicate effectively with colleagues, clients and families, and deliver client-focused care within the scope of their practice.

# Qualifications

 Preferred Qualifications (Knowledge, Experience):

  • Personal Support Worker Certificate from a recognized and accredited school.
  • Two to three years’ experience working with people experiencing homelessness or precariously housed in a community/home setting is an asset.

Essential Competencies (Skills and Attributes):

  • Demonstrated ability to provide exceptional care and customer service to clients and partners
  • Ability to work independently and function as a team member, contributing to the growth of the team.
  • Experience working with communities and individuals who face barriers accessing tradition health care institutions such as poverty, discrimination, mental health, and substance use issues.
  • An understanding of and critical analysis of the determinants of health and social factors, such as power dynamics that affect individual, organizational and community health.

Objet : Modifications au Programme fédéral de santé intérimaire (PFSI)

Madame la Ministre Diab,

Je vous écris en ma qualité de directrice générale de l’Alliance pour des communautés en santé. L’Alliance est le porte-parole d’un réseau dynamique d’organismes de soins de santé primaires interprofessionnels et gérés par la communauté. Ces organismes offrent des soins de santé complets aux personnes qui font face aux obstacles les plus importants en matière d’accès aux soins, notamment les réfugiés, les demandeurs d’asile et les nouveaux arrivants au Canada. 

Madame la Ministre, je vous adresse la présente pour vous faire part de ma profonde inquiétude concernant les modifications au Programme fédéral de santé intérimaire (PFSI) annoncées par votre gouvernement le 27 janvier dernier. Les changements prévus imposeront de nouveaux copaiements pour les médicaments et les services complémentaires (notamment le counseling, la physiothérapie, les soins de la vue et les soins dentaires) dans le cadre du PFSI. Cette politique crée des obstacles à l’accès aux soins pour les réfugiés, rappelant les mesures adoptées par le gouvernement conservateur fédéral en 2012. Même des copaiements modiques pour des médicaments et des services de santé seront inabordables pour plusieurs; certains pourraient ainsi retarder ou renoncer à des soins, ce qui risque d’entraîner des conséquences dévastatrices pour leur santé. 

La plupart des réfugiés arrivent au Canada avec des moyens financiers très limités, voire inexistants, ayant souvent fui la guerre, la persécution ou le déplacement. De nombreux réfugiés réinstallés présentent des besoins médicaux complexes ou ont subi des traumatismes, alors qu’ils ne sont pas admissibles à l’assurance-médicaments provinciale ou à l’aide sociale pendant leur première année au pays. Le PFSI offre la couverture de santé essentielle que ces personnes ne pourraient autrement s’offrir. Par conséquent, ces compressions sont d’une imprévoyance flagrante et pourraient entraîner les conséquences suivantes :

  • L’impossibilité d’obtenir des médicaments pour des soins préventifs ou aigus, ou pour traiter des maladies chroniques, comme le diabète et les cardiopathies, risque d’augmenter les visites aux urgences et les hospitalisations.
  • Les maladies infectieuses non traitées, notamment le VIH et l’hépatite, peuvent accroître les risques pour la santé tant individuelle que collective.
  • Les entraves à la couverture des soins de la vue et des lunettes ou des appareils d’aide à la mobilité peuvent nuire à la capacité d’étudier et de travailler.
  • Des infections dentaires non traitées pourraient entraîner des complications menant à des visites aux urgences et à des hospitalisations.
  • Un accès limité aux services de santé mentale peut ralentir l’intégration à la société canadienne et nuire à la capacité de contribution des nouveaux arrivants.
  • Les difficultés d’accès à la contraception et aux traitements prénataux posent des risques importants pour la santé des femmes.

Madame la Ministre, nous demandons à votre gouvernement, ainsi qu’au premier ministre, de revenir sur la décision d’imposer ces copaiements. Ceux-ci ne feront qu’alourdir le fardeau de réfugiés déjà vulnérables et, en fin de compte, augmenteront les coûts pour l’ensemble du système. Alors que d’autres pays choisissent de punir les réfugiés pour des crises dont ils ne sont pas responsables, le Canada doit rester fidèle aux valeurs de puissance moyenne que le premier ministre Carney prône souvent sur la scène internationale. Le Canada doit demeurer une terre d’accueil pour ceux qui, dans le monde, cherchent un refuge; or, cette hospitalité repose sur des politiques comme le PFSI.

En 2012, il a fallu quatre ans et un changement de gouvernement pour garantir que les réfugiés et les demandeurs d’asile puissent accéder aux soins requis afin d’éviter que leurs problèmes de santé ne s’aggravent. Il a fallu quatre ans et un changement de gouvernement pour que soit entendue la voix des médecins et des professionnels paramédicaux, regroupant 20 associations de santé et des gouvernements provinciaux de tous les horizons politiques. Il a fallu une décision de la Cour fédérale jugeant que les compressions au PFSI constituaient une violation de la Charte canadienne des droits et libertés.

Dear Minister Diab,

I am writing to you in my role as the Chief Executive Officer of the Alliance for Healthier Communities. The Alliance for Healthier Communities is the voice of a vibrant network of community-governed, inter-professional primary health care organizations who provide comprehensive healthcare to those who face the most barriers to accessing care, including refugees, asylum claimants and applicants, as well as other newcomers to Canada.

Minister, I write to you today out of deep concern for the changes to the Interim Federal Health Program (IFHP) announced by your government on January 27. The changes your government announced will put in place new co-payments for medications and supplemental services (including counselling, physiotherapy, vision, and dental care) under the Interim Federal Health Program (IFHP). The policy introduces barriers to care for refugees, echoing measures enacted by the federal Conservative government in 2012. Even modest copayments for medications and health services will be unaffordable to many, and individuals may delay or forgo care with potentially devastating health consequences. 

Most refugees arrive in Canada with very little to no financial means, often having fled war, persecution, or displacement. Many resettled refugees have complex medical needs or have suffered trauma yet are ineligible for provincial drug coverage or social assistance for one year. The IFHP provides the necessary health coverage refugees could not otherwise afford. Therefore, these cuts are incredibly shortsighted, and may lead to:

  • Failure to access medications for preventative or acute care, or to treat chronic conditions, such as diabetes and heart disease, may lead to deterioration in health conditions and increased emergency department (ED) visits and hospitalizations
  • Untreated infectious diseases, including HIV and hepatitis, may increase risks to both individual and community health
  • Barriers to coverage for vision care and eyeglasses, or for assistive mobility devices, may hinder ability to study and work
  • Unmanaged dental infections could result in complications, leading to emergency department visits and hospitalizations
  • Limited access to mental health services may slow integration into Canadian society and impede the ability to contribute
  • Difficulties accessing contraception and prenatal treatments pose significant risks to women’s health.

Minister, we call on your government, and the Prime Minister, to reverse the decision to introduce these co-payments, which will impose hardship on already vulnerable refugees and ultimately increase system costs. Other countries choose to punish refugees for crises not of their making, but Canada must hold to the middle power values Prime Minister Carney speaks of often on the world stage. Canada must remain a refuge to those in the world who need a place to go, and that door hinges on policies like the IFHP.

Back in 2012, it took four years, and a change in government, to ensure that refugees and asylum claimants could access the care they needed, to prevent medical issues from worsening. It took four years and a change in government for the voices of doctors and allied providers that encompassed 20 national health associations and provincial governments from across the political spectrum to be heard. It took a federal court decision that deemed cuts to IFHP to be a violation of the Charter of Rights and Freedoms.

This time, let’s do the right thing. Reverse this decision. Ensure that the world knows who Canada is, and that when others close their doors, ours are open to anyone in need.

 DPNCHC is a multi-service community agency providing programs and services to diverse participants who are marginalized due to socioeconomic barriers. We work from a community development and health promotion perspective.  Our Health Centre operates a busy, complex practice and is a multi-disciplinary team of physicians, nurse practitioners, nurses, counsellor/therapists, dietitian and physiotherapist. # About the position 

 Responsibilities:

  • As part of our clinical team, provide ongoing primary health care to clients within the scope of a registered practical nurse, including history taking, physical assessment, health teaching and health promotion, screening, referral education, advocacy and follow-up;
  • Practice within the standards, medical directives, and scope of practice maintained by the Centre, and the College of Nurses of Ontario;
  • Ensure appropriate, comprehensive treatment and continuity of care through maintaining complete and accurate client records;
  • Participate in primary health care program and service delivery within the broader community context in partnership with other DPNCHC services and in various community settings, such as community programs, shelters and health fairs; 
  • Help to coordinate and support health centre intake processes and conduct intakes with new patients of the health centre;
  • Incorporate best practices into care based on changing demographics and evolving needs of the community;
  • Participate in program development, delivery and evaluation and engage in committee work within the broader community context in partnership with other institutions and agencies as required. 

# Qualifications

  •  Diploma in Practical Nursing from an accredited institution;

  • Current registration with the Ontario College of Nurses in good standing;
  • Excellent oral and written communication skills in English;
  • Second language considered an asset;
  • 6 months experience post registration in a community setting;
  • Previous experience in community based primary health care within a culturally diverse community, with corresponding knowledge and commitment;
  • Strong interest in working in the community with people who are precariously housed, living with mental health and or substance use issues and who have complex health needs;
  • Strong commitment to health promotion and community development;
  • Strong commitment to practice within an inter-disciplinary environment;
  • Effective counselling and group development skills;

The Supervisor, Dental Health Programs is responsible for the direct supervision of the certified dental assistants and dental receptionists, and ensures efficient, quality, and integrated service delivery. 

Primary Responsibilities

  • Responsible for the recruitment, orientation, training, scheduling, supervision, and evaluation of the Dental Reception and Certified Dental Assistant teams, as well as applicable contract, agency, relief staff, students, and volunteers.
  • Supports the timely completion of key deliverables such as monthly reports, other reporting requirements, evaluations, denture co-payment report, and chart audits 
  • Provides input in the development and evaluation of policies, and procedures to support program enhancement and the development of quality and process initiatives
  • Contributes to overall team functioning of the dental program as whole by providing support and feedback; advisement to all dental staff regarding optimal daily operations, client flow, improved capacity and client satisfaction.
  • Works with interdisciplinary teams to collectively support client needs, and participates in activities that seek to strengthen collaborative teamwork.
  • Leads and participates in oral health promotion, presentations, outreach, advocacy, community education, and resource development in order to promote access to oral health and better serve the community
  • Performs a leadership role representing LAMP programs and objectives to stakeholders, partners, and communities served; fostering and building those relationships. 
  • Local onsite support at various clinic locations, some evening, weekends, and travel may be required.

# Qualifications

  •  Post-secondary education in health care management, health sciences, community health ore related equivalent. Dental reception or Level II Dental Assistant certification preferred.

  • Minimum of three (3) years of clinical supervisory experience within a dental environment, experience within a public health or community health environment an asset.
  • Excellent communication skills and excellent interpersonal skills to work effectively within a multi-disciplinary team, and all levels of the organization’s diverse workforce.
  • Demonstrated ability to coach and support high functioning cohesive teams.
  • Knowledge of Infection, Prevention and Control (IPAC), the Personal Health Information Protection Act (PHIPA) and the Personal Information and Electronic Documents Act (PIPEDA).
