Practical Guidance for IPCT Expansion EOI: Applying the Neighbourhood Health Home Model

In April 2025, the Alliance for Healthier Communities shared our vision for a Neighbourhood Health Home model of Primary Health Care, which enables regions to attach people to primary health care while addressing health inequities and improving primary care provider experiences. The model offers a path toward connected, equitable, and integrated care for people and communities.

Voici l'enregistrement du webinaire de lancement de la trousse de Ressources pour l'expansion des équipes interprofessionnelles de soins primaires (EISP) de l'Alliance, qui a eu lieu en octobre 2025. Supporting Access & Attachment to Primary Care: IPCT Expansion Toolkit Launch Webinar

In October 2025, the Alliance for Healthier Communities the officially launched our Interprofessional Primary Care Team (IPCT) Expansion Toolkit. In the first half of this webinar, we provided a brief overview of the Toolkit's contents, followed by a demonstration of how it might help you make the strategic and operational decisions necessary to implement your organization's expansion plans. The latter half was dedicated to a robust Q&A session with attendees. 

Webinar: Successes from Black-Focused Social Prescribing

This webinar is presented by the Alliance for Healthier Communities in partnership with participating organizations and evaluators in the Black-Focused Social Prescribing (BFSP) project.

The 2.5-year pilot was implemented by The Alliance for Healthier Communities in collaboration with four Community Health Centres (CHCs) in Ontario: Black Creek, Somerset West, TAIBU, and Rexdale. Through BFSP, dedicated staff called Link Navigators connect clients to programs and services. This project won the National Social Prescribing Innovation Award in 2024.
