This webinar is presented by the International Federation for Integrated Care (IFIC)'s Community-Enabled Integrated Care Special Interest Group (SIG) as the third in a three-part roundtable series.
#Description
From Canada, Brian Dunne and Donnie Antony from PHSS, Medical and Complex Care in Community in Ontario will share their experience of working alongside people and communities and what this means for organisational practice, relationships, implementation and decision-making.
From Buenos Aires, Gastón Perman, Hospital Italiano de Buenos Aires, Argentina, will share the story of the We Are Family programme, joined by a colleague close to its current implementation, to explore how the model developed and how community participation has shaped the work.
The conversation will look beyond programme descriptions to explore the deeper questions at the heart of community-enabled integrated care: Who has influence? Where does authority sit? How are relationships between communities and formal organisations built? What allows community agency to deepen over time? And what happens when organisational structures, funding or system expectations work against it?
Chaired by Jen Recknagel, NORC Innovation Centre, University Health Network (UHN), Canada, the session will bring the Canadian and Argentine experiences into conversation and explore what they can teach us about creating the conditions for meaningful and sustainable community partnership.
Learning from all three roundtables will help inform practical guidance on community-enabled integrated care and the SIG’s contribution to ICIC27 in Tallinn.
