As part of our series of Forum Discussions on the Nine Pillars of Integrated Care in which we explore international experiences of integrated care, we are delighted to host the eighth in our series of nine Forums to launch the revised Pillars.
Integrated care without integrated Finance tends to collapse back into fragmentation, because the underlying money flows are stronger than the goodwill of professionals trying to work across boundaries. Most health and care systems separately fund primary care, hospital care, mental health, community services, and social care, often through different commissioners, different payment mechanisms, and different accountability structures. Each silo is then rationally incentivised to optimise its own activity, costs, and risk, not the citizen’s whole journey. That is the opposite of what integrated care is trying to achieve.
This produces what is often called the "wrong pockets" problem: the sector that has to invest is rarely the one that captures the savings. Investing in age-friendly housing, community rehabilitation, or anticipatory primary care typically reduces emergency admissions and long-term residential care costs, benefits that fall to other budget holders. Without a mechanism to move money across those boundaries, the investment case rarely accrues to the party who has to spend, even when the system-wide return is strong.
