Job Summary

At the heart of our work is an interdisciplinary approach that delivers holistic, community-based care – with a strong focus on individuals who experience barriers to accessing health services. Our team is deeply committed to inclusivity and works within the social determinants of health framework, emphasizing prevention, health promotion, and client education.

Reporting to the Manager, Primary Health Care, the Registered Practical Nurse (RPN) is accountable for the provision of high-quality nursing care to clients as per the standards of the regulated profession, and in keeping with licensed requirements of the College of Nurses of Ontario. The RPN will also provide other appropriate service to clients as assigned which may include (but not limited to): administrative tasks, performing a range of medical and laboratory procedures as appropriately delegated, maintaining equipment and medical, laboratory and pharmaceutical supplies.

Primary Responsibilities

  • Clinical Assessments – Performs delegated assessments and assists with well adult, child, and well-baby exams.
  • Intake & Triage – Assists with intake, triage, routine and urgent calls, provider updates, and service navigation.
  • Complex Needs Support – Monitors patients, provides health teaching and contributes to care planning.
  • Health Promotion – Delivers preventive education, supports self-care, and identifies improvement needs.
  • Vaccine and Medical Supply – Maintains medical, pharmaceutical, biohazard and educational supplies.
  • Referrals & Procedures – Assists with referrals and specialist coordination; performs routine procedures (e.g., allergy injections; BHCG; ear irrigation; throat swabs, etc.) with proper documentation.
  • Home Visits – Provides home visits for vitals, health teaching and patient support.
  • Documentation & Quality – Maintains accurate EMR documentation, adheres to privacy requirements, and participates in chart reviews.
  • Teamwork & Confidentiality – Collaborates with the interprofessional team and supports a safe, respectful environment.
  • Other duties as assigned by the Manager      

# Qualifications

  •  Registered Practical Nurse diploma

  • Registered and in good standing as Registered Practical Nurse with the College of Nurses of Ontario

  • Minimum of two (2) years nursing experience working in community or public health environment or preferably a primary care setting

  • Superior problem-solving skills and flexible, with the ability to prioritize and time manage in a very busy office environment

  • Excellent interpersonal skills necessary to work effectively across all levels of the organization’s diverse workforce

  • Excellent communication skills, both verbal and written, with the ability to clearly convey information and ideas

  • Positive attitude and dependable with strong initiative and the ability to work both independently and in a team-oriented atmosphere

  • Open minded, eager, and willing to constantly learn and improve one-self

  • First Aid and CPR are requirements and must be kept current

  • Valid Driver’s License is an asset

  • Experience with database applications (i.e. Electronic Health Records)

  • Ability to learn new software quickly and willingness to continuously develop technology skills.

  • Demonstrated commitment to and knowledge of the principles of primary health care, community-based health care, harm reduction practices, the social determinants of health and health promotion principles and values

  • Knowledge of LAMP’s local communities, priority populations and/or a second language that is reflective of the community being served are all preferable assets.

Primary Care Access Improvement (Cohort 2)

This learning program is presented by Healthcare Excellence Canada (HEC) as part of Care Forward.

HEC is accepting applications for the next cohort of the Primary Care Access Improvement (PCAI) program This program is part of Care Forward - a growing movement to improve access to care by connecting and scaling what works. Participating teams can get up to $15000 in seed funding and are supported with expert coaching and evidence-informed tools and resources. 

# Program Overview

Across the country, primary care teams are facing growing demand, workforce pressures and increasing challenges with timely access to care. Through PCAI, team-based primary care organizations receive support, including seed funding and expert coaching, to apply Advanced  Access. This model helps clinics balance appointment availability with patient demand—optimizing clinic operations, reducing wait times, and supporting the well-being of the entire care team.

  • About the program: Primary Care Access Improvement (PCAI) helps team-based primary care organizations apply the Advanced Access model to balance appointment availability with patient demand. 
  • Why it matters: Primary care teams across Canada are facing increasing demand, workforce strain and administrative burden. PCAI supports clinics to make sustainable changes that improve patient access while strengthening provider and team well-being.
  • Impact: Early data from participating clinics shows an average wait-time reduction of seven days within the first six months. Providers from previous cohorts report improved care outcomes, reduced workloads, fewer administrative burdens and a notable increase in  overall job satisfaction. 
  • Support: Teams are supported with seed funding up to $15,000, expert coaching, and proven tools and evidence-informed resources for implementing and measuring what works. 
  • Eligibility: HEC is selecting 15 teams for this cohort. Teams must operate in a team-based model that includes multiple physicians, nurses, allied health professionals, and Medical Office Assistants (MOAs). 
  • Commitment: This 18-month commitment can result in long-term improvements in patient care, team satisfaction and clinic efficiency. To support sustainable change, teams will need to meet 30-60 minutes weekly or biweekly to test small, data-informed changes in real-time  alongside monthly coaching sessions.

Primary Care Trials Training Program

This training is provided by the Canadian Primary Care Trials Network and the Department of Family & Community Medicine at the University of Toronto.

Are you an individual interested in getting involved in clinical research, but not sure where to start? The Canadian Primary Care Trials Network (CPCTN) invites you to apply for a new, hands-on training program designed to build your confidence and skills as a site investigator in primary care trials. 

Participants in this program will receive structured guidance from initial idea through to study proposal, including one-on-one mentorship and support from an experienced clinical trialist. They will gain practical skills in clinical trial design and conduct and build confidence to participate in or lead randomized controlled trials.  

Over four months, participants will connect with peers and mentors, participate in monthly virtual touchpoints, develop a brief study proposal to apply lessons learned.

This program runs from July - October 2026.

Making Sustainability Part of Everyday Quality Improvement

This toolkit launch event is presented by CASCADES in partnership with Vancouver Coastal Health, Interior Health, and Health Quality BC.

The Sustainability-Embedded Quality Improvement (SE-QI) Toolkit is a practical resource that brings environmental sustainability into the core of quality. Co-developed through a leading collaboration between Vancouver Coastal Health (VCH), Interior Health (IH), Health Quality BC (HQBC), and CASCADES, the toolkit reflects shared expertise and real-world innovation across health systems working to deliver high-quality, low-carbon care. It supports teams to identify, measure, and report environmental impacts within new, current, or past projects, making sustainability actionable within existing QI efforts.

Tested and validated through a national pilot involving 14 healthcare organizations across six provinces in Canada, the SE-QI Toolkit has demonstrated strong usability, feasibility, and relevance in practice. The session will also introduce the SE-QI Spotlight, a growing pan-Canadian repository of real-world SE-QI examples designed to support shared learning, reduce duplication, and help teams see how sustainability can be embedded across diverse clinical and organizational contexts.

In this session, participants will be introduced to the updated Toolkit and Spotlight, hear early adoption experiences from across the country, and explore how to embed sustainability into their own improvement initiatives in a practical and meaningful way.

# About the position 

 At this time Unison is looking for a keen and committed Family Physician to provide primary care services to home-bound frail seniors in west Toronto. 

Reporting to the Hub and Satellite Manager at the Bathurst-Finch Site, this position is responsible for providing primary care services to home-bound frail seniors in west Toronto (travel area is smaller as patients are generally divided amongst the team members by geography).  The Family Physician will work with an inter-professional team of family physicians, nurse practitioner, pharmacist, care coordinator and administrative assistant to provide primary care services for seniors that are complex, frail and marginalized with medical, functional, psychosocial and mental health issues. Working with the client services team and community services partners, the incumbent will ensure that quality client-focused services are delivered.

The position is fee-for-service, but there are no overhead costs except for personal vehicle expenses (there is no compensation for transportation beyond what is covered by billed OHIP fees).  Unison provides a stipend to cover physician participation in administrative meetings. EMR (TELUS PS Suite) access, a laptop computer, and a work cell phone, as well as allied health and administrative staff salaries are covered.  

# Qualifications

 The successful candidate will have: 

  • Demonstrated experience in community-based primary care settings 
  • An interest to work with older adults and serve them in their homes
  • Geriatrics focused-practice designation an asset but not required
  • Palliative care experience an asset
  • Demonstrated knowledge of and sensitivity to the impact of social, economic, environmental and cultural issues on health 
  • Ability to work as part of an interdisciplinary team of clinical and program staff  
  • License to practice medicine and membership in good standing with the College of Physicians and Surgeons of Ontario 
  • Ability to cover other primary care providers for client care when they are not available
  • Ability to speak a second language an asset. 

The physician is compensated by billing OHIP fees.  Overhead costs are covered by Unison.

At this time, Unison is seeking a Community Health Physician for the Oakwood-Vaughan site. Reporting to the Clinical and Allied Health Site Manager for Lawrence Heights and Oakwood Vaughan, and with direction from the Clinic Coordinator, the Community Health Physician will be responsible for providing primary care to clients and their families within a multi-disciplinary, collaborative care model that emphasizes health promotion and client education.  They will also provide walk-in care to youth in areas of sexual and reproductive health, mental health and addictions. The Community Health Physician will have strong clinical and interpersonal skills to complement our team.

 General Responsibilities 

The HHP Team Lead supports the efficient and effective operations of HHP services and staff. Supervises the work of the Homeless Health Program, including both in-shelter and outreach interprofessional teams. 

Accountability 

  •  The Team Lead (Nursing) reports to the Manager of the appropriate program   

Specific responsibilities 

  • Reporting to the Manager – Clinical Community Care, the HHP Team Lead supports the efficient and effective operations of HHP services and staff.   
  • Supervises the work of the Homeless Health Program, including both in-shelter and outreach interprofessional teams, according to planning guidelines and operational policies of the Homeless Health program and WellFort Community Health Services. 
  • Acts as a professional role model, mentor and change agent for staff by demonstrating clinical expertise and evidence of ongoing professional development. 
  • Works with team members and Management to ensure adequate staffing requirements are met for both in-shelter and community outreach model, including creation of draft schedules to be reviewed and approved by leadership in alignment with the employment standards act and WellFort policies. 
  • Holds regular team meetings and huddles to keep team members appraised of Key Performance Indicators (KPI’s) and information necessary for excellent service delivery and team effectiveness. 
  • Provides clinical leadership and direction by ensuring patient care is delivered according to professional nursing standards and in accordance with policies and procedures. 
  • In collaboration with the service delivery team, contributes to the delivery of the client’s care plan. 
  • Works under delegated acts as necessary with primary care team. 
  • Partners across other service providers external to WellFort to coordinate and support integrated care. 
  • Documents problems identified using a specified framework. 
  • Selects and implements strategies or interventions from established care plans in consultation with client and/or caregiver and health care delivery team. 
  • Utilizes adult learning principles and established training materials to teach and enhance skill levels of service providers. 
  • Meets with individual team members to build education and learning plans and provides input to annual PDP. 
  • Acts as clinical resource to the service delivery team. 
  • Demonstrates effective time management, planning and organization of day’s work activities and of team members. 
  • Works collaboratively with clinical and non-clinical staff, allied health, community partners across the Peel shelter system, and other relevant partners in the advancement of the care model; ensures role clarity in the different functions of clinical and nonclinical staff. 
  • Participates in staff planning and orientation of new staff, including EMR training. 
  • Responsible for first line risk mitigation strategies for client or staff incidents. 
  • Supports the building of an integrated team of both WellFort CHS and other Community Services staff working as one team. 
  • Bridges programming between the Safe Consumption Site, Bloom program and Homeless Health programming as well as the comprehensive WellFort CHS services to support an integrated model of care. 

# Qualifications

  •  Diploma or degree in nursing from a recognized community college or university. Baccalaureate degree an asset. 

  • Registration with the College of Nurses of Ontario and in good standing. 
  • Nursing experience in a variety of care settings one of which must include community health, primary care and structurally vulnerable and homeless care. 
  • Team Lead or supervisory experienced preferred. 
  • Experience working in a Safe Consumption Site or Homeless shelter environment in the past is preferred. 
  • Coaching, mentoring and conflict resolution skills. 
  • Experience working with communities and individuals who face barriers accessing tradition health care institutions such as poverty, discrimination, mental health and substance use issues.  
  • Program planning experience considered an asset. 
  • An understanding of and critical analysis of the determinants of health and social factors, such as power dynamics that affect individual, organizational and community health. 
  • Excellent written communication and oral skills that meet the needs of the individual and/or community at the appropriate literacy level. 
  • Experience working with confidential and time sensitive documents and information. 
  • Excellent interpersonal skills and the ability to multitask in a fast paced environment. 
  • Experience working in a value based non-profit or social services organization. 
  • Demonstrated ability to work independently and interdependently in an interdisciplinary team environment in which the skills and responsibilities of each team member are recognized and respected.

Les organismes de santé du Canada reconnaissent de plus en plus la crise climatique comme étant une urgence sanitaire qui exacerbe les inégalités existantes, et appellent à plus d’action à plusieurs niveaux.  

Dans ce webinaire, l’Initiative canadienne sur la pauvreté, l’équité en santé et les changements climatiques (CPHECC) et le Centre de collaboration nationale des déterminants de la santé (CCNDS) examinent les questions souvent marginalisées dans le discours dominant sur les changements climatiques et la santé : les croisements entre pauvreté, crise climatique, santé des femmes et équité en santé.  

Un groupe d’intervenantes ayant divers points de vue parleront des systèmes qui négligent les femmes et feront part d’indications pertinentes tirées de l’expérience, de la recherche, des politiques et de la pratique. Ensemble, elles émettront une opinion critique sur la manière dont la presse écrite canadienne présente les questions des changements climatiques, de la pauvreté et de la santé des femmes, et établiront des possibilités d’agir grâce à la recherche, aux politiques et à la pratique.  

Dans le webinaire, les personnes participantes prendront part à ces sujets par sondage interactif.

The NCCDH is partnering with the Canadian Poverty, Health Equity, and Climate Change (CPHECC) initiative from the Centre for Addiction and Mental Health (CAMH) to facilitate a webinar on the connections between poverty, the climate crisis, women’s health, and health equity.

In this webinar, the Canadian Poverty, Health Equity, and Climate Change initiative (CPHECC) and the National Collaborating Centre for Determinants of Health (NCCDH) will explore issues that are often marginalized in the dominant climate change and health discourse: the intersections between poverty, the climate crisis, women’s health, and health equity.  

A group of speakers with diverse perspectives will discuss the systems that fail women, and share relevant insights from experience, research, policy, and practice.
