Peel Region. The Hub will provide primary health, mental health, addictions and social services programs that will holistically address the physical, mental, social and cultural needs of BAC communities, and seek to eliminate disparities and inequities.

 Job Summary

At the heart of our work is an interdisciplinary approach that delivers holistic, community-based care – with a strong focus on individuals who experience barriers to accessing health services.  Our team is deeply committed to inclusivity and works within the social determinants of health framework, emphasizing prevention, health promotion, and client education.

Under the direction of the Primary Health Care Manager, the Mobile Health Unit Driver is responsible for driving the mobile health unit to various clinics locations, health fairs, and community events; setting up for clinic services, and maintaining and submitting the mobile unit maintenance reports and executing related duties as assigned at clinic sites during clinic hours. We are looking for a professional, outgoing, and friendly team member with the ability to multi-task. Must be reliable, hardworking, and able to work independently. Must have basic computer skills, customer service skills, and knowledge of the Peel region and surrounding areas.

Primary Responsibilities

  • Safely load and unload medical supplies, equipment, and materials as required (up to 30–50 lbs).
  • Performs basic vehicle inspections and coordinates required maintenance or repairs to ensure the vehicle remains fully operational.
  • Maintain accurate logs for mileage, fuel usage, inspections, and service needs. Responsible for scheduling required service and repairs with the Primary Health Care Manager. 
  • Follow predefined travel routes and ensure the vehicle is fully operational at all times. 
  • Transport biohazardous specimens to labs or local public health units when required and upon return to home base. 
  • Maintain a clean driving record and valid driver’s license throughout employment period 
  • Safely transporting medical providers to and from service locations across the Region of Peel, in accordance with the vehicle schedule 
  • Maintain communication and coordination with the medical provider team throughout the day
  • Provide set-up and support functions including set up and break down of mobile clinic locations

# Qualifications

  •  Valid driver’s license eligible for use in Canada with a clean driving record for the past three (3) years. A driver’s abstract will be required upon hiring.

  • High school diploma or equivalent required 
  • Clear Vulnerable Sector Check 
  • 3+ years of professional driving experience in one of the following: patient transport (ex. Ambulance or non-emergency medical transport), courier/delivery services, or other professional driving roles with a larger vehicle such as a Benz Sprinter van (extended class).
  • Familiarity with Peel Region preferred 
  • Must submit a medical evaluation from a health care provider giving clearance to drive and assess physical ability for the job responsibilities.

The Receptionist is responsible for providing professional, welcoming, and consistent front-desk services during operating hours on a call-in basis when the regular staff is on leave or unavailable. This role ensures safe opening and closing of the facility, accurate and timely handling of clients, visitors and calls, and high-quality client service aligned with LAMP’s guiding fundamentals.

Primary Responsibilities

Front Desk and Client Services

  • Safely open and close the facility in accordance with established procedures.
  • Greet all visitors in a professional, respectful, and welcoming manner and direct them appropriately.
  • Answer incoming calls, direct inquiries to the appropriate program or staff member, and provide accurate general information.
  • Liaise with clinical and program staff (including MOAs and Intake staff) to support client flow and communication.
  • Monitor reception and waiting areas areas to ensure safety, cleanliness, and readiness for client service.
  • Respond calmly and professionally to challenging situations, using established escalation protocols and available supports (e.g. Client Response Team).
  • Maintain client confidentiality in accordance with policies and processes.
  • All other duties as assigned in line with this position

Communication and Coordination

  • Communicate operational updates (e.g. maintenance issues, service disruptions) clearly to staff using established systems.
  • Work collaboratively with other Receptionists and members of the Administration Team to ensure consistent service delivery and information sharing.
  • Participate in the Client Response Committee, as required.
  • Facilitate internal communications by posting material on bulletin boards as requested and ensure that information on the Bulletin board is kept current.

Health, Safety, and Environment

  • Monitor and restock infection control supplies at reception.
  • Maintain cleanliness of reception and common areas in line with health and safety expectations.
  • Monitor security cameras, alarms, and accessible entrances and report issues promptly.
  • Serve as the primary coordinator for fire safety by keeping the emergency binder and fire safety box in an accessible location, bringing the binder outside during an evacuation for roll call and directing emergency responders to information. 
  • Follow all organizational policies, procedures, and health and safety requirements.

Administrative and Program Support (during overlap periods) 

  • Provide administrative support to programs and administrative units as assigned.
  • Perform accurate data entry and updates as required.
  • Coordinate room bookings in accordance with policy, including internal and approved external bookings.
  • Handle mail, courier deliveries, scanning, faxing, and basic document coordination.
  • Support membership processing, donations, and coordination with Accounting as required.
  • Maintain up-to-date program and event information and ensure accurate sharing with staff and clients.

# Qualifications

  • Secondary School Diploma (or equivalent) with 1–3 years of reception experience; ability to work independently and collaboratively in a fast-paced environment.

  • Demonstrated experience providing exceptional customer service and fostering a welcoming, professional environment with strong client relations.
  • Excellent interpersonal and communication skills (verbal and written) to work effectively across all levels of a diverse workforce.
  • Experience working with vulnerable populations, including exposure to crisis situations, de-escalation, or similar settings, is an asset.
  • Strong organizational skills with the ability to manage competing priorities, remain flexible, and stay calm under pressure while meeting timelines.
  • Alignment with the organization’s values and goals; availability to work weekends as required, with knowledge of the local community and/or a second language considered an asset

 WellFort is seeking a Dentist to join our Health n’ Smiles team!  Health n’ Smiles provides services to under resourced families living in the Region of Peel. Our team includes Denturists, Dental Assistants and Dental Hygienists.

The Dentist provides oral health care to their patients. This care includes providing preventative care and patient education on teeth, gum, and mouth care. The Dentist is responsible for examining the teeth, gums and mouth as well as a performing a variety of dental procedures including, but certainly not limited to, tooth extractions and fillings. The Dentist can perform additional services for their patients, and along with that comes even more responsibility to ensure proper care for their patients. The Dentist is often also responsible for properly training and supervising staff. 

Responsibilities: 

  • Performs complete oral examinations, formulates comprehensive treatment plans, informs clients of and obtains consent for treatment plans including hygiene. 
  • Performs recall exams and works in conjunction with hygiene team to ensure comprehensive oral health of clients on a regular basis. 
  • Performs dental treatments such as diagnostic, restorations, crown and bridge, denture services, endodontic, periodontal, and preventative care. 
  • Performs universal precaution procedures for infection control. 
  • Prescribes antibiotics and administers local anesthetics. 
  • Responsible for fabrication of dental prosthetic appliances either individually or in conjunction with the denturist. 
  • Consultation with other health professionals as required in establishing a diagnosis and plan of care. 
  • Participating in the development, implementation, monitoring and evaluation of treatment, education, counseling, and health promotion services for individuals, families, and the community. 
  • Providing services to clients at outreach locations. Maintains contact as required by Clinical Protocols with the physician or other staff through telephone consultation. 
  • Other duties and responsibilities as may be assigned from time to time. 

# Qualifications

  •  Degree in Doctor of Dental Surgery, licensed and in good standing with RCDSO 

  • Extensive experience in a large dental practice. 
  • Experience working in a community dental clinic or in a public health environment is an asset.

 At this time Unison is seeking a keen and committed Nurse Practitioner-Registered Nurse, Extended Class – RN (EC) for our Lawrence Heights site.  The RN (EC) diagnoses and manages common health problems and diseases as part of the multidisciplinary, primary health care team.

Job-Specific Responsibilities:  

  • Perform clinical duties within the scope of practice, including assessments, diagnosis, treatment, follow up, counseling, screening, education, and referral. 
  • Collaborate with primary care physicians to focus on clients that are homebound and unable to access primary care services.
  • Provide consultation and support related to clinical care to front line staff and our partners.
  • Provide feedback on performance and care standards of staff and care providers.
  • Provide supervision and clinical oversight including application of corrective and remedial actions, as required.
  • Provide education/training, knowledge management expertise and research as required.
  • Build and maintain relationships with health and social service agencies and external service providers.

Organizational Responsibilities:

  • Participate in team and organization-wide meetings and committees, initiatives and events.
  • Develop work goals in conjunction with supervisor, identifying action plans, learning objectives and professional development needs. 
  • Participate in performance appraisal process including self-evaluation component.
  • Adhere to all policies and procedures of the organization.
  • Submit encounter data and/or statistics, administrative forms and reports in a timely fashion.
  • Orient, supervise and support students and/or volunteers as required. 
  • Identify, report and debrief on client safety incidents, good catches, occupational health and safety incidents and near misses.
  • Other duties as required.

# Qualifications

  •  Registration in the Extended Class with the College of Nurses of Ontario. 

  • Sound clinical judgment and strong health assessment and diagnostic skills.
  • Demonstrated ability to work effectively independently and as a member of a multi-disciplinary team.
  • 3-5 years’ experience in a community-based setting or nursing experience in Primary Care/NP.
  • Excellent triage and clinical skills. 
  • Willingness to teach /work with other professional disciplines and students. 
  • Demonstrated knowledge and experience in group work and community development.
  • Demonstrated knowledge of, and sensitivity to, the impact of social, economic, environmental and cultural issues on diverse communities.
  • Knowledge of issues affecting communities facing barriers to access in an urban environment.
  • Familiarity with the social, economic and cultural conditions of the health centre’s area; experience in working with local service providers, community groups and networks, and an understanding of local issues and priorities are an asset.

  • Demonstrated commitment to equity goals and principles, to sexual and reproductive rights, to being youth positive, LGBTQ2IA+ positive and sex positive and to being inclusive of diverse sexual and gender identities.

# About the position 

 The Garden & Food Access Facilitator is responsible to plan, facilitate and coordinate garden programs and food access services primarily at Harmony Community Food Centre (CFC) 2 Gower Street, and at partner sites.  This position is expected to work onsite.

Responsibilities:

  • Meet with Program Manager, Community Health and other CFC and/or CHC staff on a regular basis to identify programming needs, challenges and opportunities
  • In collaboration with Program Manager, Community Health and other CFC staff develop and implement a workplan for the weekly seasonal Affordable Produce Market (APM) program
  • Lead the weekly Seasonal Affordable Produce Market [APM] with support from CFC staff and Summer Youth 
  • Lead and collaborate with other CFC staff to conduct outreach to promote the seasonal Affordable Produce Market (APM)
  • Develop and implement monthly work plans for the community garden pilot program, food access workshops and garden club/group in consultation with staff and group participants, where needed 
  • In coordination with community group leads, plan and facilitate innovative, participatory workshop sessions on food or garden related topics to be delivered in workshop room, HH, or 955 Queen St. E. rooftop garden, local school garden or community garden
  • Conduct outreach to promote the food access and garden programs
  • Ensure proper use and operation of composter, irrigation system, pollinator garden boxes, vertical gardens and other gardening-related tools and supplies
  • Work with program staff to appropriately document garden programs and community garden use (narrative, stats and photos), and ensure records are up to date on a monthly basis
  • Secure written permission from clients/staff prior to taking photos for communications strategies
  • Ensure session rooms are set-up before and appropriately cleaned-up after each session
  • Coordinate snack preparation for each gardening group with Kitchen Facilitator
  • Where available, work with summer youth to support gardening programs and activities
  • Conduct evaluation of the garden programs with participants and program staff 
  • Prepare and submit reports to Program Manager as required by funding 
  • Support work involving food access, including meal preparation, community meals, SALC congregate dining and with Food Connect
  • Complete SRCHC mandatory training, or as deemed appropriate to better support the group
  • All other duties as required

# Qualifications

  •  Post-secondary education in a relevant discipline or equivalent in work experience 

  • Minimum 2 years’ experience working with marginalized populations
  • Food handler’s certification is an asset
  • Skilled and experienced in food access work, including affordable produce markets
  • Skilled and experienced in planning and delivering evidence-based urban gardening programs, including rooftop gardens
  • Strong understanding of group dynamics and process with an emphasis on primary principles of confidentiality, trust building and working knowledge of anti-oppression non-discrimination values 
  • Experience working with diverse populations and ages, from children to adults, including youth and seniors
  • Demonstrated respect for diversity, equity, anti-racism and accessibility with the ability to work collaboratively and positively with others
  • Proficiency in the use of computer technology and various software applications
  • Strong organizational skills and ability to multitask
  • Excellent verbal and written communication skills; fluency in English (read, write, speak)

The Diabetes Educator (Registered Dietitian) is a key member of a multidisciplinary team of dietitians, chiropodists and nurses providing education and support for adults (18+) living with Type 2 Diabetes, Prediabetes, and other chronic illnesses. Working in collaboration with primary care providers and diabetes specialists, the Diabetes Educator position emphasizes advanced clinical decision making, client teaching and chronic disease management. The Educator serves as a resource and consultant for clients and community partners, integrating population health approaches with direct care.  This role involves interpreting clinical data, using lifestyle management to support diabetes care and staying current with diabetes care standards and technology (i.e. continuous glucose monitoring, insulin pumps etc.). The role carries a caseload and provides individualized counselling and group education on topics such as self-management, nutrition, physical activity, stress management, blood glucose monitoring, and medication management. The role empowers clients to set and achieve personalized health goals for optimal diabetes care. 

Program:

  • West Toronto Diabetes Education Program: A comprehensive diabetes education program, designed to improve the quality of life for clients 18+ living with Type 2 Diabetes or Prediabetes and at risk.

Primary Responsibilities

  • Provide assessment and counselling for clients living with pre-diabetes and Type 2 diabetes, taking into consideration client’s level of knowledge, readiness to change, medical and non-medical factors and socio-demographic background.
  • Develop health improvement plan in partnership with the client by integrating information about health status, clinical practice guidelines, and available community resources.
  • Ensure complete, accurate, and timely documentation of program activities using electronic medical records.
  • Plan, develop and facilitate health education sessions on various topics related to diabetes in both individual and group settings.
  • Participate in the development and improvement of team and organizational procedures, workflows and policies as assigned.
  • Identify and build relationships with various community partners.
  • Other duties as assigned by Manager.

# Qualifications

  •  Bachelor’s degree in Nutrition and in good standing with the College of Dietitians of Ontario as a RD.

  • Completion of an accredited Dietetic Internship Program; preferably with a community focus.
  • Certified Diabetes Educator (CDE) credentials or current enrollment would be an asset.
  • Excellent communication skills, including the ability to communicate information and ideas to support understanding with diverse demographics, newcomers, and individuals with language barriers
  • Strong knowledge of diabetes pathophysiology, related laboratory values, and pharmacotherapy.
  • Experience with facilitating diabetes education and behaviour change counselling; with a Nutrition focus.
  • Demonstrated ability to teach and facilitate groups using principles of adult learning.
  • Positive attitude and dependable with strong initiative and the ability to work both independently and in a team-oriented atmosphere.
  • Food Handlers certificate required.
  • First Aid and BLS certification required and must be kept current.
  • Valid Driver’s License is an asset.
