Alliance Social Prescribing and Health Promotion Professional Learning Event
This in-person learning event is hosted by the Alliance for Healthier Communities.
This in-person learning event is hosted by the Alliance for Healthier Communities.
This fall, the Alliance will host our third Learning Collaborative: Increasing efficiency to improve access to care.
On September 28, 2023, from 12-1 pm, the Alliance’s Learning Health System team will host a Lunch & Learn to share information about our upcoming learning collaborative, Increasing Efficiency to Improve Access to Care, and to answer questions about the opportunity.
Climate change is a significant and growing driver of health inequities even here in Canada. However, there are cost-effective ways for primary health care organizations to mitigate the inequitable impacts of climate change on the health of their clients and communities.
We cordially invite you to join us in person for Connected Communities: Building Equitable Integrated Healthcare. Alliance's Primary Health Care Conference will focus on ways to centre community voices and needs, and how to build foundations on comprehensive primary health care, health promotion, chronic disease prevention and management, and actively addressing the determinants of health (housing, income, food security, etc.).

This includes providing tailored data reports and curated tools and resources; hosting peer-supported learning activities, such as learning collaboratives and communities of practice; developing self-directed learning programs such as RALI and the Online Social Prescribing Course; and offering direct QI and implementation coaching tailored to your organization's setting and learning goals.
This webinar is presented by the Alliance for Healthier Communities in Partnership with CareNow Ontario.
Are you seeing clients with persistent but hard-to-explain symptoms — debilitating fatigue, dizziness, unrefreshing sleep, pain, or brain fog — and wondering how best to help when tests don’t provide clear answers?

This QIP season, the Alliance is introducing a new Common QIP Indicator for our sector: "Number of New Clients/Patients."
This new indicator will replace "Client Involvement in Decisions about their Care," which Ontario Health discontinued in 2025. It was chosen by Alliance members through the EPIC Learning Health System Steering Committee, and their technical specifications were developed in partnership with the Improvers Working Group.
