All interprofessional primary health care organizations, as well as hospitals and long-term care facilities, are required to submit a quality improvement plan (QIP) to Ontario Health (OH) every year. QIPs are due for the upcoming fiscal year on April 1.
QIPs are submitted to OH through their online QIP Navigator tool (now open for 2026-27).
Each year, OH selects several priority issues for the QIP program and a number of suggested indicators for each of these issues. Although it is mandatory to submit a QIP, the suggested indicators are optional for primary health care organizations. Each organization should tailor or choose indicators according to their strategic goals as well as the priorities, needs, and assets of the people and communities they serve.
Where possible, every organization's QIP should reflect quality improvement work that is already planned or underway there. Additionally, Alliance member organizations are strongly encouraged to include the Common QIP Indicators that were selected by our members.

Common QIP Indicators for Alliance Member Organizations
Since 2019, we have encouraged our members to start from a set of Common QIP indicators developed and ratified by our membership. These common QIP indicators are informed by our sector's core values, as reflected in our shared Model of Health and Wellbeing, our Health Equity Charter and our collective QI efforts. The list of indicators has changed over time, with guidance from Alliance members, in response to shifting sector and health system priorities.
The Common QIP indicators for 2026-27 are:
- Completion of Sociodemographic Data Collection
- Stratified Cervical Cancer Screening Rate
- Client Feels Comfortable and Welcome at CHC
- Client Perception of Timely Access to Care
- (NEW) Number of New Clients/Patients

# New for 2026-27: Number of New Clients/Patients
Number of New Clients/Patients is a new common QIP indicator selected by Alliance members for 2026-2027 from the OH priority list for our sector. It replaces Client Involvement in Care Decisions, which was removed from the OH indicator matrix in 2025. This new indicator is aligned with the broader system objective of reaching full attachment to team-based primary care.

# Accessing Your Data for the Common QIP Indicators
For each indicator in your QIP, there is a "current performance" field.
Data for three of the Common QIP indicators (Completion of Sociodemographic Data Collection, Stratified Cervical Cancer Screening Rate, and Number of New Clients/Patients) comes from your EMR. This data is aggregated by the Alliance through our Business Intelligence Reporting Tool (BIRT) and compiled in a Common QIP Data Report. This report is shared with executive leaders of our member CHCs each year in February. If you are an EL at a CHC and have not received the most recent report, please email EPIC@AllianceON.org to request it.
Ontario Health's 2026-27 QIP Priority Issues, Indicators, and Supports
Ontario Health (OH) has selected 13 matrix indicators for Interprofessional Primary Care for 2026-27. These are grouped into three quality domains: Access & Flow, Equity, and Safety, as listed below. Indicators in boldface are OH Priorities for 2026-27. Indicators marked with an asterisk (*) are also found in the Alliance's Common QIP indicators.

- * Patient/client perception of timely access to care (priority)
- * Number of new patients/clients/enrolments (priority)
- Percentage of clients with type 2 diabetes mellitus who are up to date with HbA1c (glycated hemoglobin) blood glucose monitoring
