# About the organization
Access Alliance Multicultural Health and Community Services (AAMHCS) is a Community Health Centre that aims to provide services and addresses system inequities to improve health outcomes for the most vulnerable immigrants, refugees, and their communities. The Centre envisions a future in which Toronto’s diverse communities achieve health with dignity. Access Alliance is a member of the Mid-West Toronto Ontario Health Team (OHT), and the lead agency for the Open Door Program (ODP) which is a health initiative focused on improving outcomes for vulnerable populations in priority neighbourhoods. The program increases access to health information and services by supporting preventive care, connecting clients to primary care, and linking individuals to mental health, addiction, and community services.
This role is well suited for nurses who are interested in community-based care, including those early in their careers who have gained relevant experience through placements, practicums and in team-based settings.
You will join a collaborative, interdisciplinary team that includes nurses, a community health worker, community health ambassadors, and other professionals. The Nurse plays a key role in delivering care, supporting clients, and working with community partners to improve access and continuity of services.
# About the position
Responsibilities:
- Provide nursing care to clients of all ages within the full RN scope of practice mostly in outreach, and community settings.
- Support individuals facing complex health and social challenges to access primary care and related services.
- Deliver health promotion and education initiatives tailored to diverse communities.
- Develop and maintain educational materials and program resources.
- Lead implementation of the Preventive Care Program, including planning and delivery of project activities.
- Monitor and evaluate program outcomes, and recommend improvements using quality improvement approaches.
- Provide guidance and support to health ambassadors and project staff within a team-based model.
- Build and maintain partnerships with community agencies to support referrals and coordinated care.
- Establish and maintain clear referral pathways with partner organizations.
- Work with the supervisor to develop work plans, learning goals, and professional development objectives.
- Collect and submit program data and reports in a timely manner.
- Work evenings and weekends as required.
# Qualifications
Required:
- Bachelor Degree of Nursing.
- Current registration with the College of Nurses of Ontario.

Minimum 2 years of experience in frontline nursing and implementation of evidence informed practice.
Preferred:

Experience in a community health centre and/or family practice setting preferred.

Thorough familiarity with therapeutic methods and practices based upon a health promotion/disease prevention model.

Ability to work collaboratively and in a team based setting with multiple provider types and independently when necessary.

Demonstrated commitment to principles of an anti-oppression and cultural competence framework.

Thorough understanding of health promotion and community development principles.

Demonstrated experience in helping clients navigate the health care system and build capacity for self-management.
- Experience developing and evaluating patient education materials and programs.

Comfort using health technologies and digital tools.

Excellent interpersonal, communication (written and verbal), problem-solving, organizational and time management skills.

An interest and commitment to working with low-income, multilingual, multiracial communities.

Thorough knowledge of broad determinants of health and issues affecting low income, multi-lingual and racialized and 2SLGBTQI+ communities.
Asset:

Experience providing care or education in community or non-clinical settings.

Experience facilitating group education sessions.

Experience in program development, monitoring and evaluation.
- Training in Motivational Interviewing and preventive care planning.
- Experience in partnership development and report writing.
