LAMP Community Health Centre (LAMP CHC) is partnering with Roots Community Services Inc (RootsCS) and

the Canadian Mental Health Association Peel Dufferin (CMHA Peel Dufferin) to design, develop and implement an integrated health and social services Hub with and for the Black, African and Caribbean (BAC) communities in

Peel Region. The Hub will provide primary health, mental health, addictions and social services programs that will holistically address the physical, mental, social and cultural needs of BAC communities, and seek to eliminate disparities and inequities.

 Job Summary

Reporting to the Manager, Primary Health Care, the Diabetes Nurse Educator (DNE) is accountable for the provision of high-quality nursing care to clients as per the standards of the regulated profession, and in keeping with licensed requirements of the College of Nurses of Ontario. The Diabetes Nurse Educator is a key member of a multidisciplinary team of dietitians, chiropodists and nurses providing education and support for adults (18+) living with Type 2 Diabetes, Prediabetes, and other chronic illnesses. Working in collaboration with primary care providers and diabetes specialists, the nurse educator position emphasizes advanced clinical decision making, client teaching and chronic disease management. The DNE serves as a resource and consultant for clients and community partners, integrating population health approaches with direct care. This role involves interpreting clinical data, supporting medication management in collaboration with prescribers, and staying current with diabetes care standards and technology (i.e. continuous glucose monitoring, insulin pumps etc). The role carries a caseload and provides individualized counselling and group education on topics such as self-management, nutrition, physical activity, stress management, blood glucose monitoring, and medication management. The role empowers clients to set and achieve personalized health goals for optimal diabetes care.

Primary Responsibilities

  • Provide assessment and counselling for clients living with pre-diabetes and Type 2 diabetes, taking into consideration client’s level of knowledge, readiness to change, medical and non-medical factors and socio-demographic background.
  • Develop health improvement plan in partnership with the client by integrating information about health status, clinical practice guidelines, and available community resources.
  • Ensure complete, accurate, and timely documentation of program activities using electronic medical 
  • records.
  • Plan, develop and facilitate health education sessions on various topics related to diabetes in both individual and group settings.
  • Participate in the development and improvement of team and organizational procedures, workflows and policies as assigned.
  • Identify and build relationships with various community partners.
  • Maintain accurate, timely EMR documentation; adheres to privacy policies; participates in chart and case reviews and quality initiatives.
  • Collaborate with nurse practitioners, physicians, social workers, dietitians, and others to support a safe, respectful, and equitable care environment.
  • Other duties as assigned by Manager

# Qualifications

  • Bachelor of Science in Nursing (BScN) and in good standing with the College of Nurses of Ontario (CNO) as an RN;

  • Certified Diabetes Educator (CDE) credentials or current enrollment would be an asset.
  • Excellent communication skills, including the ability to communicate information and ideas to support understanding with diverse demographics, newcomers, and individuals with language barriers
  • Strong knowledge of diabetes pathophysiology, related laboratory values, and pharmacotherapy.
  • Experience with insulin titration, oral medication management and use of medical directives is an asset.
  • Demonstrated ability to teach and facilitate groups using principles of adult learning.
  • Positive attitude and dependable with strong initiative and the ability to work both independently and in a team-oriented atmosphere.
  • First Aid and BLS certification required and must be kept current.
  • Valid Driver’s License is an asset.
  • Experience with database applications (i.e. Electronic Health Records)
  • Ability to learn new software quickly and willingness to continuously develop technology skills.
  • Demonstrated commitment to and knowledge of the principles of primary health care, community-based health care, harm reduction practices, the social determinants of health and health promotion principles and values 
  • Familiarity with Peel Region and experience serving African, Caribbean and Black communities preferred
