# About the position 

 Reporting to the Manager, HEQHR, the Policy Analyst will collaborate with cross‑departmental staff, provide member and committee support, and engage with stakeholders to advance racial justice and human rights within the organization and across the midwifery sector. They will also conduct research, develop content, and drive the education and promotion of racial equity and human rights in midwifery to achieve the Association of Ontario Midwives’ (AOM) Strategic Plan

The ideal candidate brings lived experience with systemic racism and a strong foundation in health or public policy, with demonstrated expertise in research, policy development, and health equity. They bring strong communication (written and oral), analytical, and relationship‑building skills, apply anti‑racism and anti‑oppression principles with cultural humility, and can effectively manage multiple priorities in a fast-paced environment while navigating complex and challenging issues. 

Position Responsibilities: 

Shared Responsibilities 

  • Provides culturally safe service and employ humility when working collaboratively across the organization, particularly with the Indigenous Midwifery (IM) and Health-Care Equity, Quality and Human Rights (HEQHR) departments 
  • Supports a workplace that is respectful and inclusive; and specifically, works to fulfil the expectations described in the AOM’s Anti-Oppression, Diversity, Equity and Inclusion Policy 
  • Engages in organizational trainings and opportunities to promote cultural safety and positive work environments within and across teams 
  • Works collaboratively within and across teams to ensure high quality programs and services for members 
  • Provides support to other members of the staff team during periods of high workload 
  • Supports a workplace that is healthy and safe by engaging in health and safety activities and monitoring the workplace for and reporting hazards to physical and mental health 
  • Attends and participates in staff and team meetings 
  • Develops an annual work plan with their manager or director, in the context of the AOM strategic plan 
  • Ensures coverage of AOM core programs/services during vacations/leaves of other staff 
  • Assists with AOM conference or conference-related work, as requested 
  • Contributes to content for communications or publications (this will include a variety of communication tools and strategies, such as web content, member emails and content for either member or public consumption) as requested 
  • Monitors and contributes updates for the AOM web site as required 
  • Ensures all external communications comply with AOM Communications Standards  
  • Ensures appropriate level security of information stored within the AOM 

Specific Responsibilities 

Member Support and Stakeholder Engagement 

  • Maintains a working relationship with members and stakeholders 
  • Staff lead on working groups and projects as assigned 
  • Responds to member calls/emails to provide member support on all matters related to HEQHR work, including but not limited to racism and inequity in the workplace and in the provision of care, supporting clients experiencing racism or inequity, developing an antiracism strategy for the practice/hospital/birth centre, and implementing anti-racism processes etc.  
  • Conducts community engagement and consultation to engage with and learn from those directly impacted by racism and human rights-based inequities to identify members’ needs and deliver programs and services that are responsive to the needs of such communities  
  • Drives work to operationalize the objectives of committees, task forces and working groups, providing support to the Director, Chair and midwifery members of those groups as required  

Policy Analysis 

  • Supports a portfolio of policy topics, responding to developments and initiating work on any of those topics as the need arises 
  • Undertakes research and provides summaries on key issues and policy directives  
  • Develops and/or delivers educational content including website resources, online learning, and live events to promote racial justice and human rights 
  • Prepares policy papers, briefing notes, background summaries, and recommendations on key policy issues impacting midwives and the AOM 
  • Contributes to the analysis of policy issues within a context of diversity, equity, inclusion (DEI) and human rights 
  • Assists with writing grant proposals, reports to the AOM Board, MOHLTC, briefing notes, etc. 
  • Assists the Manager in developing an annual plan for work groups/committees and ensures that deliverables are met 
  • Performs other tasks as assigned 

Cross-Departmental work 

  • Works collaboratively across AOM departments to support staff to incorporate principles of racial justice and human rights into the various programs and services provided by the AOM, to ensure they are reflective of the needs of Indigenous, Black, racialized and equity-deserving communities 
  • Supports the development of knowledge translation materials and resources which reflect the racial and cultural demographics that provide and require midwifery care  
  • Supports the provision of risk management guidance that centres antiracism and anti-oppression to address systemic racism in healthcare 
  • Performs other tasks as assigned. 

# Qualifications

  • University degree in midwifery (preferred), health care, public policy, or related field; or professional equivalency 
  • Master’s degree in public policy or health is an asset  
  • Experience working as a midwife in Ontario is an asset   
  • Lived experience with systemic racism, including anti-Black racism and/or human rights-based discrimination  
  • Demonstrated understanding of research and policy development; preferably of health policy development within an association environment  
  • Strong written and oral communication skills, with the ability to communicate respectfully and with cultural humility using an anti-racism and anti-oppression approach. 
  • Understanding of perinatal health in Ontario, health (in)equity, and strategies to address systemic racism and human rights inequity are an asset 
  • Strong understanding of the intersectional nature of the social and cultural determinants of health as it relates to racism, particularly, anti-Black racism  
  • Ability to work in a self-directed manner on own initiative as well as collaboratively within the team and with cross-departmental staff 
  • Strong interpersonal skills and attributes, including humility, maturity, diplomacy, and the ability to create positive, professional relationships with AOM staff, members, and stakeholders  
  • Strong attention to detail with high levels of accuracy  
  • Ability to effectively manage multiple priorities and demands in a fast-paced environment while navigating complex and challenging issues 
  • Highly proficient with Microsoft Office (e.g., Word, Excel, PowerPoint, Outlook) 

 

Location: This position is remote; however, the successful candidate will be required to work on-site as per departmental needs.

Duration and Work Hours: This is a full-time (35-hour work week), 12-month contract position with the potential for an extension or a permanent position, subject to departmental needs. 

Expected start date: June 2026.

Salary: The salary range for this position is $70,144 to $105,216 per year with an expected starting salary of approximately $70,144 to $87,680, depending on qualifications.

  •  Brief and short-term trauma-informed counselling for clients. 

  • Support newcomers with mental health needs in any context; culturally appropriate approaches; trauma informed practice; integrating promising and emerging practices. 
  • Client care coordination, including orientation, assessment, referrals to internal and external resources, case management and discharge planning.
  • Participate actively in the Centre's interdisciplinary team, supporting integrated approaches to client services based on social determinants of health.
  • Establish and maintain links with organizations and groups in the community.

 Required

  • Minimum 3 years full time work experience in clinical counselling.
  • Thorough familiarity and work experience with therapeutic methods and practices based upon a health promotion model.
  • Excellent case management, casework, counselling, crisis intervention, group facilitation skills.
  • Travel within Toronto and work flexible shifts including evenings and Saturdays. 

Preferred

  • Excellent networking and partnership development skills and interest in health promotion and research.
  • Strong computer literacy (word processing, internet, data entry in Windows applications, electronic medical record charting).

# About the position 

Position Summary: 

Community Health Ambassadors work on meeting and speaking with Fairview residents in one-on one conversation with community members about chronic disease, mental health resources, cancer screening and health service navigation. Community Health Ambassadors (CHA) will support the above-mentioned initiatives across the M3C, M4H, M2J, M2M, M2N, M2R and M3H neighbourhoods, including local retail spaces, high density neighbourhoods, faith-based spaces, vulnerable housing clients, appointment accompaniment, vaccine on-site clinic support and virtual community engagement opportunities.  

  • Provide up-to-date general information about Flemingdon Health Centre and services available through East Toronto Health Partners. 

  • Attend all training(s) and information sessions hosted by service providers 

  • Hours determined as needed in designated building for direct tenant engagement. 

  • Support and lead multi-week building by building and door by door approaches in the M3C, M4H, M2J, M2M, M2N, M2R and M3H  neighbourhoods 

  • Attending monthly meetings, completing quarterly reports and other duties assigned as assigned 

  • Regular face-to-face or virtual interactions or group info sessions with residents

  • Ensure that all communications and records be kept confidential and secure 

  • Establish and maintain relationships with landlord, superintendent, building manager(s), tenants, and      tenant groups – upload data to progress tracking tools 

  • Assist to note, update and maintain accurate information and records 

  • Other duties as assigned by Agency Staff Leads  

# Qualifications

  •  Experienced in working with newcomers, racialized and/or vulnerable populations, providing frontline services and outreach 

  • Excellent motivational communication skills and exceptionally strong organizational skills 

  • Ability to understand and respond to changing community needs 

  • Ability to work independently and collaboratively as part of teams, in a fast-paced environment 

  • Excellent written and spoken English communication skills (using devices and computers) 

  • Multicultural sensitivity and/or experience working in diverse neighborhoods like Flemingdon Park 

  • Familiarity in working from an anti-oppression, LGBTQ+ positive framework

Position Summary

The Case Worker supports Social Worker clients with intake and case management using a person-centered, approach. Case Workers are responsible for developing comprehensive needs assessments with individuals who are experiencing concerns related to mental health, substance use, and poverty, and assisting individuals in navigating Flemingdon programs and external systems providers to acquire healthcare, housing, education, identification, income, and other resources. Flemingdon’s approach requires proactive engagement to “meet people where they are.” Flemindgon Health Centre’s consent-based, relationship-building approach seeks to address the root causes to promote long-term wellness and safety.

Responsibilities include:

General

  • Provide culturally sensitive services to clients in person and virtually

  • Complete accurate documentation, including in Flemingdon’s Electronic Medical Record, Practice Solutions (PSS)

  • Provides reports as needed/required

Intake – New Clients

  • Provide intake assessment and support to people living in the catchment area.

  • Screen, enroll and orient new clients to the health centre services and programs.

  • Provide information and supportive services to other referring agents, and liaise closely with other service providers to ensure optimum delivery of services.

Navigation and Case Management Support

  • Meets regularly with individual clients to monitor needs and progress on an ongoing basis, provide case 

  • management, referrals, mentoring, and coaching

 Position Summary: The Social Worker will work as part of a community-based interdisciplinary team to provide mental health and psychosocial care, therapeutic counselling, case management and follow-up support to clients within the context of a client-centred care model and under the established standards of professional practice. ABOUT THE RPCHC BOARD OF DIRECTORS RPCHC is governed by a Board of Directors elected from our membership. The Board plays a governance role – it is not directly involved in operational decisions and matters, or other decisions that have been delegated to staff. The Board generally meets 8 times per year and has an Annual General Meeting (AGM) where the membership and the community are invited and encouraged to attend. Remuneration Directors of the RPCHC Board do not receive any remuneration. This is a volunteer position.

Terms of Service Each Director is elected to hold office for a four-year term and may be eligible for re-election three successive times.

Board Committees The Board has several committees designed to provide it with advice. The committees are chaired by a member of the Board but may include other volunteers. These committees include:

  • Quality Committee – Providing advice and oversight on the quality, safety and equitable access to services to ensure that community needs and regulatory standards are met.

  • Governance committee – Providing advice and leadership to help the Board be successful in its job of governing, including recruiting and nominating new Board members.

  • Directors are not directly involved in operational decisions and matters, or other decisions that have been properly delegated to staff.

Conflict of Interest Directors shall take care to ensure that they identify, avoid and take steps to guard against even the appearance of a conflict. A Director or a Board committee member is encouraged to discuss any outside business activity with the Chair of the Board or Board committee that could create a potential conflict. All Directors are asked to sign a conflict-of-interest form and are asked to declare any conflicts at the beginning of all meetings.

 The RPCHC Board is seeking various qualified candidates. Given the existing board complement and existing organizational priorities, we are particularly interested in candidates with some demonstrated history of board governance and/or significant volunteer experience, and expertise in one or more of the following areas:  

  • Financial management with a particular interest in health system experience (accounting designation preferred)

  • Performance and quality management in healthcare settings

  • Human resources management and diversity, equity, and inclusion expertise

  • Communications, public affairs, and government relations

  • Fundraising, resource mobilization and partnerships

  • Community engagement and development of community hubs  

In addition to the above, we are always interested in potential candidates in the Regent Park community (as a resident and/or someone with significant ties to the community).
