ConnectWell Community Health is a fully accredited, community governed, and not-for-profit organization. ConnectWell provides community-based primary care, health promotion programs and services, and a range of developmental and social services across Lanark, Renfrew, and Leeds and Grenville Communities. ConnectWell Community Health services are provided by salaried interdisciplinary team members, working to their fullest possible scope, in a collaborative practice, to address the health and social well-being to residences. A strong understanding and willingness to learn about social determinants of health needs of the people and communities we serve. 

# About the position 

The Primary Care Nurse Practitioner (NP) functions as a member of the Primary Care Team in the provision of primary health care services offered at ConnectWell Community Health. Primary health care services are provided at ConnectWell and in the community, with an emphasis on health promotion, client education and self-management. The RN (EC) must be aware of, understand, and adhere to the Standards of Practice set out by the College of Nurses of Ontario

# Qualifications

•  Completion of recognized Primary Care Nurse Practitioner program 

• Masters degree in Nursing, or equivalent recognized education and experience 
• Current and unrestricted registration as an RN (EC) with the College of Nurses of Ontario 
• Two years experience in the nurse practitioner role in a primary care setting or community preferred 
• Demonstrated ability to work effectively in an inter-disciplinary team 
• Demonstrated leadership in promoting a shared responsibility for professional practice and development 
• Ability to work evening hours, as required 
• Ability to provide on-call telephone services on a rotation basis 
• Valid driver’s license and use of a vehicle
