# About the position 

 Summary:

The Physician provides primary health care in a team environment. The Physician is an integral member of an interdisciplinary team whose objective is to provide comprehensive health care and follow up.  The position has a strong emphasis on health promotion, disease prevention, and community capacity building and works within a harm reduction framework.

# Key Objectives:

  1. Provide primary health care to individuals who experience barriers to accessing health care. 
  2. Deliver primary health care in the community health centre as well as other health care settings, community-based points of service, and through home visits. 
  3. Ensure appropriate, comprehensive treatment is provided by maintaining complete and accurate charts/medical records and participate in chart reviews and case conferences.

  4. Complete intake and coordinate treatment plans. 
  5. Liaise with hospital system to facilitate and support continuity of care.
  6. Act as part of an interdisciplinary team which assists with the development, analysis and implementation of policies and procedures for the benefit of improvement to the delivery of primary health care services. 
  7. Develop and maintain strong ties to other professionals and community members.
  8. Participate in and share responsibility for the on-call coverage offered by Quest CHC.
  9. Assist with the development, analysis, implementation and evaluation of policies and procedures.
  10. Supervise student/learners by providing competent leadership, orientation, developing and scheduling of training/learning plans including shared client contact.

  11. Provide input for development of strategic plans and attend and participate in staff meetings and special projects as assigned.

  12. Conduct presentations, facilitate workshops and group meetings as well as participate in program planning and community initiatives.
  13. Participate in the Integrated Community Lead (ICL) Model, which includes participating in community meetings, organizing and hosting, and follow up for the ICL Model for complex care needs clients (e.g. Health Links clients).
  14. Work within the full scope of practice. Meet organizational quality and quantity of work standards.  Comply with applicable medical directives, policies, regulations and legislation identifying any areas of noncompliance. 
  15. Participate in, and comply with, all health and safety processes and initiatives to ensure the organization remains a safe workplace.
  16. Maintain and develop professional competence through training or appropriate continuing education.

  17. Promote the mission, vision, values and philosophy of care developed by Quest CHC.
  18. Other duties may be assigned as required.

# Qualifications

 Education         

  • Current degree in Family Medicine from a recognized university and currently licensed to Practice in the province of Ontario by the College of Physicians and Surgeons of Ontario. 
  • Certificate of the College of Family Practice of Canada.
  • Membership in good standing of the Canadian Medical Protective Association (CMPA), with liability coverage.

Experience

  • Experience supporting individuals without a healthcare provider who are also experiencing mental health, addictions or concurrent disorders, street-involved populations (homeless, under-housed, sex trade workers), isolated seniors, sexually and gender diverse populations (2SLGBTQ+), at-risk children, youth and families.
  • Experience in sexual health and/or HIV-related care is an asset.
  • Experience in program development, implementation, monitoring and evaluation. 
  • Experience working with interdisciplinary teams is an asset.
  • Experience in working with hazardous chemicals and materials and associated preventive measures outline/required by product Safety Data Sheets and WHMIS

Skills and Abilities

  • Awareness of, and sensitivity to, the health issues of the community.
  • Familiar with Community Health Centre model of needs and wellbeing.
  • Ability to provide compassionate, respectful, non-judgmental and culturally competent care to priority populations.
  • Thorough knowledge and proficiency in current primary health care services, in a community setting.
  • Knowledge or willingness to learn approaches to care, including harm reduction, trauma informed, client driven care, and motivational interviewing.
  • Knowledge of sexual health and HIV-related care or willingness to build capacity in this area.
  • Strong interview skills and ability to communicate effectively in English, both in writing and orally. Proficiency in other languages is an asset.
  • Ability to function independently in potentially difficult situations and the ability to decide what presenting conditions are a priority.
  • Ability to deal with disruptive behaviour, crisis management and de-escalation.
  • Ability to plan and organize assigned duties.
  • Ability to establish positive working relationships with multi-disciplinary team, clients and service providers.
  • Ability to problem-solve and apply sound judgment. 
  • Proficiency in the use of computer software such as Microsoft Office and electronic medical records such as PS Suites.
  • Knowledgeable with resources within the community and methods of access.

Conditions of Employment

  • Valid Ontario Driver’s License, access to a reliable vehicle, and a Police Reference Check are required. 
  • Current certification or willingness to become certified in Non-Violent Crisis Intervention (NVCI).
  • Maintain confidentiality in compliance with the Personal Health Information Protection Act (PHIPA), Freedom of Information and Protection of Privacy Act (FIPPA) and Personal Information Protection and Electronic Documents Act (PIPEDA).
  •  Maintain and develop professional competency through training or appropriate continuing education.

# About the position 

 Key Responsibilities include:

  1. Advocating on the behalf of clients, participating in the development, implementation, monitoring and evaluation of care programs and services for individuals, families and communities.

  2. Working with clients to identify and improve their movement and function. Helping to promote their clients' health and wellbeing, and assisting in the rehabilitation process by developing and restoring body systems, in particular the neuromuscular, musculoskeletal, cardiovascular and respiratory systems.

  3. Developing and reviewing individually tailored treatment plans.

  4. Treating a wide variety of conditions, such as injuries (including sports injuries), fractures, orthopaedics, post-surgical rehabilitation, arthritis, strokes, musculoskeletal conditions, women’s health, posture and movement, neurological conditions. Treatment involves encouraging exercise and movement with the use of techniques such as therapeutic exercise and movement, massage, manual therapy and modalities for pain control and functional improvement.

  5. Charting in a clinic wide electronic medical record, participating in chart reviews and making the appropriate referrals

  6. Educating and advising clients and care providers on prevention and/or ways to improve conditions

  7. Liaising with an interdisciplinary team to supply and receive relevant information about the background and progress of clients, as well as referring patients who require other specific medical attention.

# Qualifications

Education and Experience Required:

  1. Current registration and in good standing with the College of Physiotherapists of Ontario.

  2. Three to five years of experience in an outpatient rehab setting would be an asset.

  3. Experience in Physiotherapy program development, implementation, monitoring and evaluation

  4. Demonstrated experience working in a multidisciplinary team.

  5. Commitment to and understanding of the Community Health Centre model of health and well-being.

  6. Advanced proficiency in the use of computers and various software applications including Microsoft Office and electronic medical records (PracticeSolutions experience would be considered as asset).

  7. GLA:D training, pelvic floor training or experience/training for physio in Pregnancy, would be an asset

  8. Experience working with diverse populations and equity focused initiatives is an asset

  9. Working knowledge in more than one language is an asset.

Déclaration sur le budget de l’Ontario : Renforcer les soins de santé primaires et accroître le rattachement

L’Alliance pour des communautés en santé salue les progrès continus réalisés pour consolider les soins de santé primaires à l’échelle de la province, ainsi que les mesures prévues pour élargir l’accès aux soins, dont celles présentées dans le dernier budget provincial. Nous sommes ravis d’annoncer que bon nombre de nos membres ont déjà reçu de nouveaux financements, et nous félicitons le gouvernement pour ses avancées, alors que la province vise à ce que 100 % de la population soit rattachée à une équipe de soins primaires, garantissant ainsi l’accès à un fournisseur de soins pour chaque Ontarienne et chaque Ontarien d’ici 2029. 

L’Alliance a toujours pour priorité fondamentale de renforcer les liens entre les soins de santé primaires et les services communautaires. Le modèle de maison de santé de proximité (également appelé maison de santé de quartier) est au cœur de cette démarche. Ce modèle, que nos membres ont mis en œuvre et qu’ils incitent le gouvernement à adopter pour arrimer les soins cliniques aux services communautaires, permet d’agir sur les déterminants sociaux de la santé et d’offrir des services adaptés aux réalités locales. Nous pressons le gouvernement d’inscrire le soutien au modèle des maisons de santé de proximité dans le Plan d’action pour les soins primaires. Cela permettra d’accroître le taux de rattachement - et donc l’accès à un fournisseur de soins primaires - tout en réduisant les coûts hospitaliers évitables pour les contribuables. 

En complément des maisons de santé de proximité, nous réclamons des investissements ciblés pour les agents de liaison [étude de cas]. Ces derniers servent de trait d’union entre les soins primaires et les ressources communautaires en aidant les gens à accéder au logement, aux mesures de soutien du revenu, aux soins de santé mentale et de toxicomanie, ainsi qu’à d’autres programmes sociaux favorisant de meilleurs résultats en matière de santé. Il est essentiel de déployer et de pérenniser le rôle des agents de liaison pour alléger la pression sur les équipes de soins primaires et les services d’urgence de la province, tout en veillant à ce que la population reçoive des soins holistiques axés sur la communauté. 

Les membres de notre Alliance continueront de soutenir les initiatives visant à maintenir ces fonds au sein de la communauté et à obtenir des investissements supplémentaires dans les logements avec services de soutien. Il s’agit d’un complément indispensable aux maisons de santé de proximité et aux efforts déployés pour rattacher les patients à un fournisseur de soins primaires. 

L’Alliance souhaite poursuivre sa collaboration avec le gouvernement de l’Ontario afin de resserrer les liens entre les soins primaires et la santé communautaire, et ce, grâce au modèle de maison de santé de proximité, aux agents de liaison et à des investissements durables axés sur l’équité.

Statement on the Ontario Budget: Strengthening Primary Health Care and Expanding Attachment

The Alliance for Healthier Communities welcomes continued progress in strengthening primary health care across the province, along with the supports outlined to expand access to care, including those introduced through the last provincial budget. We are pleased to share that many Alliance members have already received new funding, and we commend the government’s progress as the province works toward 100 per cent attachment to primary care by 2029. 

Our priorities as an alliance remain focused on strengthening the connection between primary health care and community services. The Neighbourhood Health Home model is central to that work. The model that our members have implemented and have called on the government to integrate clinical care with community services addresses social determinants of health and ensures services are locally responsive. We urge the government to embed support for the Neighbourhood Health Homes model in the Primary Care Action Plan to increase attachment and reduce unnecessary hospital costs for taxpayers.  

In parallel with the Neighbourhood Health Home, we have been advocating for dedicated investments in Link Workers [case study]. Link Workers bridge primary care and community resources, helping people access housing, income supports, mental health and substance use services, and other social supports that drive better health outcomes. Scaling and sustaining Link Workers is essential to relieving pressure on primary care teams and emergency departments across the province and to ensure people receive holistic, community-centred care.  

Our Alliance members will continue to support efforts to ensure those funds remain in the community and to secure additional investment in supportive housing, an essential complement to the Neighbourhood Health Home and to efforts to attach people to primary care. 

The Alliance looks forward to working with the provincial government to strengthen the connection between primary health care and community health through the Neighbourhood Health Home model, Link Workers and sustained, equity-focused investments.
