Medication optimization: Opportunities for action in pharmacy and prescribing | Pharmacy, Prescribing and Climate Action webinar series

THis webinar is presented by CASCADES Canada as part of their Pharmacy, Prescribing and Climate Action series.

Medications are a necessary tool in healthcare. They also contribute to climate change and environmental pollution, some of the biggest health challenges of our time. Medication optimization is an important opportunity to reduce the harms to patients, health systems, and the environment caused by inappropriate medication use—and can play a vital role in shaping a healthier, more sustainable future.

On Tuesday, October 7th, join Sarah Fallis (RPh), Lisa McCarthy (RPh), and Myles Sergeant (MD) for an engaging session exploring the environmental footprint of medications and the exciting future of deprescribing. This dynamic team will share real stories of innovative medication optimization projects happening across the country.

Come be part of the conversation on building a greener, healthier path forward in pharmacy and prescribing.

CASCADES’ Pharmacy, Prescribing and Climate Action webinar series is designed to address the increasing challenges the pharmacy profession is facing and explore opportunities for change. It is organized in partnership with the Centre for Practice Excellence at the University of Toronto Leslie Dan Faculty of Pharmacy, Dalhousie University’s College of Pharmacy in the Faculty of Health, the Faculty of Pharmaceutical Sciences at the University of British Columbia and other university partners.

ECHO Planetary Health & Sustainable Care ECHO - Fall 2025 session

This ECHO Learning Series is presented by the Collaborative Centre for Climate, Health & Sustainable Care in partnership with CASCADES and the Canadian Institute for Social Prescribing.

Planetary Health & Sustainable Care ECHO is a collaborative space for people in the Canadian healthcare sector to discuss, troubleshoot, and celebrate efforts to provide climate resilient and responsible care across settings and specialties. The next ECHO cycle, titled Integrated Care for Sustainable Health Systems: Highlighting Innovative Approaches, will run on Tuesdays, 12-1 pm, from October 14 to December 2, 2025.

Registration is open on a rolling basis, meaning those interested can register at any time before and throughout the cycle and attend any number of sessions. Registrants will receive a calendar invitation to the entire cycle (all 8 sessions).

The upcoming cycle, will explore how integrated models of care—particularly those that bridge clinical services with social supports—can advance both human and planetary health. Strategies such as social prescribing, improved access to primary care providers, co-locating providers, and care coordination help address the social determinants of health. By shifting care upstream and connecting patients with appropriate medical services and community-based resources, these approaches improve health outcomes and reduce reliance on resource-intensive, hospital-based care—contributing to a lower carbon footprint in healthcare delivery. Additionally, by fostering stronger social infrastructure and more equitable access to care, these systems build climate-resilient communities—ones better equipped to withstand and recover from climate-related shocks like extreme heat and wildfires.

With a particular focus on social prescribing, this cycle will highlight how integrated care models promote environmental sustainability by reducing unnecessary interventions, transportation, and waste, while simultaneously improving population health outcomes and climate resilience. Through short lectures and case presentations, we will consider the many ways in which strengthening the connection between medical care and social support can lead to a healthier, safer, more sustainable future for both people and the planet.

This cycle is done in partnership with the Canadian Institute for Social Prescribing (CISP).

Launch Webinar: Interprofessional Primary Care Team (IPCT) Expansion Toolkit

In response to the challenges faced by people in Ontario accessing primary health care, the Ontario Government has made a historic investment in Interprofessional Primary Care Team (IPCT) expansion. Under the leadership of Dr. Jane Philpott, teams have been funded to expand with a goal of increasing attachment for people in Ontario. An additional call for expressions of interest will be issued this fall.

The Alliance for Healthier Communities has created an online IPCT Expansion toolkit to support organizations who received this funding, as well as any IPCTs looking to increase their operational capacity and advance primary care access and attachment in their communities.

This toolkit consists of resources, case examples, and change ideas organized across nine key themes. It is available for free on our website, in English and French, for anyone who wishes to access it.

We hope that that primary health care providers, teams, and decision-makers across all models and serving communities all over Ontario will find it valuable.

Join us on Monday, October 6, from 12-1 pm, for the official launch of our IPCT Expansion Toolkit. We'll provide a brief overview of the Toolkit's contents, followed by a demonstration of how it might help you make the strategic and operational decisions necessary to implement your organization's expansion plans. We'll conclude with a Q&A session for anyone wanting to know more.

We hope you can join us live; however, the webinar will be recorded and shared with anyone who is unable to attend.

This webinar will be presented in English with simultaneous French-language interpretation.

Planning for Implementation Practice Workshop

This worksho is presented by the SickKids Knowledge Translation Program

About PIP™.

The Planning for Implementation Practice© (PIP) workshop offers a clear and practical introduction to implementation planning that is applicable across innovations, settings, and sectors. Designed for those working in health, mental health, education, and allied sectors, this workshop introduces participants to the core elements of evidence-based implementation using a structured, evidence-informed, case-based approach.

You will not be planning your own implementation endeavour in this workshop, as this is something you will need to do with your implementation teams after the workshop (implementation planning is a group activity conducted by an implementation team in the implementing setting).

Participants will be guided through The Implementation Roadmap© (TIR) - a planning tool grounded in implementation science - by working through a detailed, real-world case example. This case-based format allows participants to explore each phase of the Roadmap, reflect on how the concepts apply in practice, and build confidence in using the TIR approach. Each participant will receive a copy of the TIR© Workbook to support future implementation planning in their settings

Patients as partners in Planetary Health and Sustainable Care

This webianr is presented by CASCADES Canada.

How can we meaningfully engage patients and people with lived experience in healthcare sustainability efforts?

Canadians are concerned about climate change and many patients express enthusiasm for being able to respond to the climate crisis through their care. Patients experience the health impacts of climate change, whether that be through increases to their care needs or reduced capacity of the healthcare system due to heat, wildfires, and other extreme weather events. Working with patients to implement both mitigation and adaptation strategies ensures that these solutions consider perspectives that healthcare providers and leaders may overlook.

Join us on October 2nd as we explore and elevate the role of patient and community partners in advancing planetary health and sustainable care, by sharing principles, real-world case studies, and strategies for meaningful engagement across Canadian health systems.

Presented in partnership by National Health Engagement Network (NHEN) and Creating a Sustainable Canadian Health System in a Climate Crisis (CASCADES).

Beyond sustainability: The power of Indigenous healthy energy homes

This webinar is presented by the National Collaborating Centre for Environmental Health

The Healthy Energy Homes project, a partnership between Indigenous Clean Energy and the Canadian Climate Institute, aims to address the long-standing challenge that many Indigenous people in Canada live in inadequate housing. Poor quality homes are causing severe health impacts in Indigenous communities across Canada—investing in Healthy Energy Homes could help address these impacts, make communities more resilient to climate impacts, and help reduce emissions and health care costs.

This webinar will focus on the project's first report, Beyond Sustainability: The Power of Indigenous Healthy Energy Homes, which outlines the connections between housing, health outcomes, and energy efficiency. The webinar will highlight both the barriers and innovative community-led solutions being led across the country.

Storytelling Workshop

This in-person workshop was developed by Renira Narrandes and is being hosted by the SickKids KT Program at the Hostpital for Sick Children (SickKids).

# About the Workshop.

Storytelling is a powerful knowledge-sharing tool that can help you connect with your audience, get your message across, and be memorable-at conferences, retreats, meetings, and anywhere you speak about your work. It’s also useful when describing the impact of your work to funders and the general public, or when developing knowledge translation products, such as videos.

# We will:

  • Explore the art of storytelling as a method to communicate your work, engage others, and inspire action
  • Examine real examples of stories that have been developed based on scientific projects and annual reports
  • Debunk five myths of science storytelling
  • Learn how to prepare, structure, and tell a story well
  • Discuss the importance of considering/measuring a story’s impact
