Ontario's vision for connected primary care depends on more than expanding access. It requires strong relationships between primary care organizations, Ontario Health Teams, community partners, specialists and hospitals that are built around the needs of local communities and the full population of people within them.
Join leaders from Southlake Academic Family Health Team, the Northern York South Simcoe Ontario Health Team, and Southlake Health as they share how they are building a neighbourhood-based model of integrated care across 6 unique communities in the Northern York South Simcoe region.
Rather than creating new structures, their approach strengthens the relationships that already exist, connecting primary care, community services, specialists and hospital care into a coordinated network that will support providers to create better experiences for all local residents via guaranteed attachment and improved access to primary care.
Participants will hear practical lessons on developing the partnerships, governance and shared approaches needed to create a strong foundation for integrated care while maintaining the unique strengths of each organization.
Whether your community is beginning this work or looking to strengthen existing partnerships, this webinar will provide practical ideas that can be adapted to your local context.
# Participants will have the opportunity to:
- Hear how Southlake Health, Southlake Academic Family Health Team and the Northern York South Simcoe Ontario Health Team are working together to strengthen neighbourhood-based partnerships across primary care, community organizations, specialists and hospital care that will ensure timely and responsive access to care for everyone.
- Explore practical approaches to building trust, strengthening relationships and creating the conditions for meaningful collaboration and shared accountability across organizations with different mandates, priorities and cultures.
- Reflect on how existing partnerships can evolve into stronger neighbourhood networks that better support patients, providers and communities.
- Consider questions, ideas and emerging practices that can help inform your own community's approach to designing more connected, integrated systems of care that are accountable to the community and everyone in it. Engage in a candid discussion about the opportunities, challenges and lessons emerging from this work, and how they may apply in different local contexts across Ontario.
