Depuis plus de 50 ans, les centres de santé communautaire (CSC) de l’Ontario font la prestation de soins de santé primaires complets selon les principes communs décrits dans le Modèle de santé et de bien-être. À l’instar d’une maison de quartier, nos organisations membres vont au-delà de leurs portes pour améliorer la vie des gens dans leurs communautés.
For over 50 years, community health centres (CHCs) in Ontario have delivered comprehensive primary health care according to the shared principles described in our Model of Health and Wellbeing (MHWB). Like a neighbourhood home, our member organizations reach beyond their doors to improve the lives of people in their communities. Although it began with CHCs, this approach is now embodied by organizations from multiple primary health care models, who together form the Alliance for Healthier Communities.
Public health surveillance involves the collection, analysis, interpretation, and sharing of health-related data to support public health decisions and actions. This essential function enables public health organizations to safeguard the health and safety of people residing in Canada by detecting emerging threats, guiding appropriate responses, and informing prevention strategies.
Challenging Compounding Disadvantage is a community-based research project that explored the compounded effects of the COVID-19 pandemic on immigrants and refugees living with cancer or mental health and addiction disorders.

This report describes the complex, generalist roles of social workers in primary health care settings and demonstrates how this work advances the goals of primary health care. It describes challenges experienced by social workers in primary care settings and outlines a vision for the future in which social workers are embedded in all primary care teams and accessible to everyone in Canada; social workers work to full scope and are equitably compensated; and the value of social work in primary care is clearly demonstrated through robust data collection and linkage.
