Compensation
The Alliance is conducting a project to measure the impact of chronic disease management health promotion programs on client outcomes in CHCs, to 1) demonstrate the importance of health promotion as part of our model of care, and 2) support the creation of a standardized approach to evaluation health promotion programming. This project was co-designed with CHC staff and CHC client advisory groups. The Alliance will obtain consent and conduct all participant surveys and interviews. After data collection is complete, CHCs will be provided with their site-specific evaluation results.
# What’s involved?
- CHC health promotion staff: Complete a program description survey and support client recruitment.
- CHC health promotion clients: Complete short 5-minute pre- and post-program surveys and a 6-month follow-up. There is also an optional 30–45-minute phone interview.
# Who can participate?
# Eligible health promotion programs must:
- Have a goal of improving knowledge or skill development in management of chronic disease
- Be time-limited (e.g., 4 weeks, 3 months, etc.)
- Conclude by the end of August 2026
# Eligible clients must:
- Be participating in the health promotion program for the first time
- Have a chronic disease/illness
Interpretation services are available for participants to complete the surveys in the language of their choice.
# Compensation
Clients will receive a $25 gift card after completing the pre-and post-program surveys. Clients will receive an additional $25 gift card if they are invited for an optional 30–45-minute virtual interview.
