UHN OpenLab is conducting a quality improvement project to better understand how primary care providers support complex patients after discharge from highly specialized hospital care, starting with transplant recipients.

Ontario primary care physicians and nurse practitioners are invited to participate in a 30–60 minute virtual interview about experiences with post-discharge communication, role clarity between primary and specialty care, medication/prescription issues, follow-up, escalation pathways, and what would help primary care teams support complex patients in the community.

Participants will receive a $50 honorarium. Findings will be used to inform co-design of practical supports for better shared care between hospital specialty teams and primary care.
