#About the position 

SUMMARY

This position is a shared role between the primary care clinic and the regional diabetes education program at RCHS, Brockville site.  Within the primary care clinic, the Registered Nurse (RN) provides direct client care, nursing services, health education, and care coordination to individuals with complex needs within a multidisciplinary primary care environment. The RN independently leads and delivers specialty clinics and provides clinical support to the primary care clinic as required to ensure continuity, accessibility, and safety of care. This role performs clinical and coordination functions to support the delivery of comprehensive, client‑centred primary health care.  Within the diabetes education program, the RN works alongside a team of RNs and registered dietitians to provide community-based nursing focused on diabetes education and management, and participates in the development, delivery and evaluation of programs and services for individuals and families that are affected by diabetes in Lanark, Leeds and Grenville.

REPRESENTATIVE ACTIVITIES

• Provides comprehensive nursing care to complex clients using an interdisciplinary, team‑based approach within both specialty clinics and the primary care clinic.
• Independently plans, implements, and evaluates specialty clinics in alignment with organizational priorities, clinical guidelines, and client needs.
• Supports the primary care clinic by providing nursing coverage, assessment, triage, and clinical interventions as needed to ensure continuity of care.
• Conducts nursing assessments, determines urgency and complexity of client needs, and performs appropriate triage and referral.
• Delivers health education, health promotion, and disease prevention services to individuals and groups, tailored to identified needs.
• Works closely with clients, dietitians, primary care providers, social services and health service agencies to provide diabetes education and management support to adults with diabetes within Lanark, Leeds and Grenville.
• Provide medication and insulin recommendations to primary care providers based on Diabetes Canada Clinical Practice Guidelines when the appropriate knowledge, skill and judgement has been demonstrated (eg, completion of CDE certification)
• Counsel clients on insulin initiation, adjustments and pattern management when the appropriate knowledge, skill and judgement has been demonstrated (eg, completion of CDE certification)
• Coordinates care and services across internal programs and external providers to support integrated and timely client care.
• Supervises and supports nursing students during clinical placements at the Centre.
• Participates in health centre meetings and contributes to the development, review, and refinement of clinical protocols, workflows, and procedures.

#Qualifications

 TYPICAL QUALIFICATIONS:

• BScN with current Registration with the College of Nurses of Ontario.
• Experience with complex clients (rostered clients, community clients and post discharge from hospital). 
• Three to five years nursing experience; experience within a community setting or in the development and delivery of health education programs is an asset.
• Certified Diabetes Educator certification is an asset.
• Strong assessment and clinical skills.
• Excellent communication skills to function within a multidisciplinary team.
• Strong organizational skills to thrive in fast paced environment.

• Adheres to Occupational Health and Safety policies and procedures at all times.

• Ability to work flexible hours based on program needs
• Valid Driver’s license and vehicle to travel between RCHS locations as needed.
