With new funding to expand our role as a health hub in North Durham, we are growing our team and enhancing services so individuals and families can access the care they need close to home in a supportive, inclusive setting.

#About the position 

Brock CHC is seeking a Chiropodist who shares our passion for community-focused health, and is motivated to make a lasting difference in a rural setting where their work will truly matter.  This position will have an important role in improving access to foot care services for patients who may experience barriers to accessing specialized care, including older adults, individuals living with chronic conditions, and people experiencing social or economic barriers to health care.

Key Responsibilities

• Conduct comprehensive assessments of foot and lower-limb conditions
• Diagnose and treat conditions within the Chiropodist's scope of practice
• Provide treatment and management of common foot conditions, including skin and nail disorders, corns, calluses, warts, infections and other foot-related concerns
• Provide diabetic foot assessments, education, prevention and management in collaboration with the primary care team
• Identify patients at risk of foot complications and provide appropriate intervention, education, referrals and follow-up
• Develop individualized treatment and care plans based on patient needs and goals
• Provide education related to foot health, footwear, mobility, prevention and self-management
• Maintain accurate, timely and confidential medical documentation in accordance with professional and organizational standards
• Strive to achieve provincial performance targets set by Ontario Health
• Promote a team-based, integrated approach to primary care that considers the patient’s physical, mental, social and environmental health needs
• Work collaboratively with physicians, nurse practitioners, nurses, medical secretaries, allied health professionals and other members of the interdisciplinary CHC team

#Qualifications

•  Graduate of an accredited Chiropody program

• Current registration and good standing with the College of Chiropodists of Ontario
• Eligible to practice as a Chiropodist in Ontario
• Alignment with the Centre’s values – equity, confidentiality, compassion, collaboration, responsiveness, education and respect
• Experience as a Chiropodist providing comprehensive foot care and assessment is required
• Experience working with patients with diabetes, vascular disease, mobility issues or other chronic conditions is an asset
• Experience working in rural, vulnerable or medically complex populations (preferred)
• Experience working in a primary care, community health, or interprofessional setting is an asset
• Proficiency with EMR systems
• Commitment to patient-centred, equitable and accessible health care
• Strong time management and organizational skills
