Dear Minister Diab,

I am writing to you in my role as the Chief Executive Officer of the Alliance for Healthier Communities. The Alliance for Healthier Communities is the voice of a vibrant network of community-governed, inter-professional primary health care organizations who provide comprehensive healthcare to those who face the most barriers to accessing care, including refugees, asylum claimants and applicants, as well as other newcomers to Canada.

Minister, I write to you today out of deep concern for the changes to the Interim Federal Health Program (IFHP) announced by your government on January 27. The changes your government announced will put in place new co-payments for medications and supplemental services (including counselling, physiotherapy, vision, and dental care) under the Interim Federal Health Program (IFHP). The policy introduces barriers to care for refugees, echoing measures enacted by the federal Conservative government in 2012. Even modest copayments for medications and health services will be unaffordable to many, and individuals may delay or forgo care with potentially devastating health consequences. 

Most refugees arrive in Canada with very little to no financial means, often having fled war, persecution, or displacement. Many resettled refugees have complex medical needs or have suffered trauma yet are ineligible for provincial drug coverage or social assistance for one year. The IFHP provides the necessary health coverage refugees could not otherwise afford. Therefore, these cuts are incredibly shortsighted, and may lead to:

  • Failure to access medications for preventative or acute care, or to treat chronic conditions, such as diabetes and heart disease, may lead to deterioration in health conditions and increased emergency department (ED) visits and hospitalizations
  • Untreated infectious diseases, including HIV and hepatitis, may increase risks to both individual and community health
  • Barriers to coverage for vision care and eyeglasses, or for assistive mobility devices, may hinder ability to study and work
  • Unmanaged dental infections could result in complications, leading to emergency department visits and hospitalizations
  • Limited access to mental health services may slow integration into Canadian society and impede the ability to contribute
  • Difficulties accessing contraception and prenatal treatments pose significant risks to women’s health.

Minister, we call on your government, and the Prime Minister, to reverse the decision to introduce these co-payments, which will impose hardship on already vulnerable refugees and ultimately increase system costs. Other countries choose to punish refugees for crises not of their making, but Canada must hold to the middle power values Prime Minister Carney speaks of often on the world stage. Canada must remain a refuge to those in the world who need a place to go, and that door hinges on policies like the IFHP.

Back in 2012, it took four years, and a change in government, to ensure that refugees and asylum claimants could access the care they needed, to prevent medical issues from worsening. It took four years and a change in government for the voices of doctors and allied providers that encompassed 20 national health associations and provincial governments from across the political spectrum to be heard. It took a federal court decision that deemed cuts to IFHP to be a violation of the Charter of Rights and Freedoms.

This time, let’s do the right thing. Reverse this decision. Ensure that the world knows who Canada is, and that when others close their doors, ours are open to anyone in need.

We are looking for a professional, outgoing, and friendly team member with the ability to multi-task. Must be reliable, hardworking, and able to work independently. Must have basic computer skills, customer service skills, and knowledge of the Peel region and surrounding areas.

Primary Responsibilities

  • Safely load and unload medical supplies, equipment, and materials as required (up to 30–50 lbs).
  • Performs basic vehicle inspections and coordinates required maintenance or repairs to ensure the vehicle remains fully operational.
  • Maintain accurate logs for mileage, fuel usage, inspections, and service needs. Responsible for scheduling required service and repairs with the Primary Health Care Manager. 
  • Follow predefined travel routes and ensure the vehicle is fully operational at all times. 
  • Transport biohazardous specimens to labs or local public health units when required and upon return to home base. 
  • Maintain a clean driving record and valid driver’s license throughout employment period 
  • Safely transporting medical providers to and from service locations across the Region of Peel, in accordance with the vehicle schedule

The Diabetes Educator (Registered Kinesiologist) is a key member of a multidisciplinary team of dietitians, chiropodists and nurses providing education and support for adults (18+) living with Type 2 Diabetes, Prediabetes, and other chronic illnesses. Working in collaboration with primary care providers and diabetes specialists, the Diabetes Educator position emphasizes advanced clinical decision making, client teaching and chronic disease management. The Educator serves as a resource and consultant for clients and community partners, integrating population health approaches with direct care.  This role involves interpreting clinical data, using lifestyle management to support diabetes care and staying current with diabetes care standards and technology (i.e. continuous glucose monitoring, insulin pumps etc.). The role carries a caseload and provides individualized counselling and group education on topics such as self-management, nutrition, physical activity, stress management, blood glucose monitoring, and medication management. The role empowers clients to set and achieve personalized health goals for optimal diabetes care. 

Program:

  • West Toronto Diabetes Education Program: A comprehensive diabetes education program, designed to improve the quality of life for clients 18+ living with Type 2 Diabetes or Prediabetes and at risk.

Primary Responsibilities

  • Provide assessment and counselling for clients living with pre-diabetes and Type 2 diabetes, taking into consideration client’s level of knowledge, readiness to change, medical and non-medical factors and socio-demographic background.
  • Develop health improvement plan in partnership with the client by integrating information about health status, clinical practice guidelines, and available community resources.
  • Ensure complete, accurate, and timely documentation of program activities using electronic medical records.
  • Plan, develop and facilitate health education sessions on various topics related to diabetes in both individual and group settings.

# Qualifications

  •  Bachelor’s degree in Kinesiology and Health Science/ Human Kinetics and in good standing with the College of Kinesiologists of Ontario (COKO) as a R. Kin.

  • Certified Diabetes Educator (CDE) credentials or current enrollment would be an asset.
  • Excellent communication skills, including the ability to communicate information and ideas to support understanding with diverse demographics, newcomers, and individuals with language barriers
  • Strong knowledge of diabetes pathophysiology, related laboratory values, and pharmacotherapy.
  • Experience with facilitating diabetes education and behaviour change counselling; with an Exercise focus.

Reporting:

  • Assist with the supervision of program participants.
  • Assist with outreach for children and youth programs.

  • Assist with maintaining and uploading programs and resources to our children and youth Facebook page, you tube and Instagram.

Families and Community:

 Position Summary: 

The Social Worker will work as part of a community-based interdisciplinary team to provide mental health and psychosocial care, therapeutic counselling, case management and follow-up support to clients within the context of a client-centered care model and under the established standards of professional practice. We are seeking a Social Worker who is committed to strength-based care, and passionate about working collaboratively to address the social determinants of health, to reduce barriers and improve access to community resources and comprehensive health care.

Main Tasks and Responsibilities

Client Care

  • Provide assessment and counselling services with individuals and families including children, youth, and adults.

  • Jointly work with the client and interdisciplinary team to develop a service plan outlining therapeutic goals appropriate to the client’s needs and issues.

  • Provide information, referrals, basic case management and client advocacy when needed.

  • Facilitate client linkages with internal/external resources and effectively coordinate client supports.

  • Attend and/or coordinate case conferences as needed.

  • Maintain clinical treatment records for each client and prepare verbal and written reports and correspondence related to the work.

Group Work and Community Development

  • Conduct health education/promotion workshops on mental health and wellness and other issues based on the needs of the community. 

  • Participate in community-based groups or committee relating to Social Determinants of Health.

  • Participate in outreach activities as needed. 

  • Conduct and complete group program evaluations.

Administrative and Organizational Responsibilities

  • Participate in the planning, implementation, and evaluation of Social Work goals. 

  • Participate in committees, team meeting and working groups. 

  • Participate in continuing education activities and assist in the in-service training of other employees.

  • Participate in continuous quality improvement initiatives.

  • Support student and volunteer placement programs as required.

  • Prepare statistical and annual reports as required.

# Qualifications

 Master’s degree in social work (MSW) preferred.

  • Registered and in good standing with the Ontario College of Social Workers and Social Services Workers.

  • Clinical social work expertise and mental health care with 3 -5 years of experience in a community based, health or social service organization located in a culturally diverse setting.

  • Experience providing clinical counselling and group programming to children, youth and adults.

  • Knowledge and proficiency in social work assessment, trauma-informed care and evidence-based therapeutic counselling models including Cognitive Behaviour Therapy (CBT), Dialectical Behaviour Therapy (DBT), Brief/Solution Focused Therapy and Single Session Therapy (SST).

  • Knowledge and proficiency in culturally competent counselling and social work services.

  • Knowledge of and commitment to anti‐racist and anti-oppressive social work theory and practice.

  • Knowledge of community resources, income security programs, food security resources, and subsidized housing sectors.

  • Ability to build a professional network with other Social Workers - in the sector and connect with resources as needed for programs and services.

  • Experience working with newcomers to Canada, refugees and people whose first language is not English.

  • Experience working with economically and socially marginalized populations with complex mental health care needs.

  • Knowledge and experience in community development, social action, health promotion and community outreach.

  • Strong conflict resolution, crisis management and problem-solving skills.

  • Excellent communication, interpersonal, and organizational skills; effective team player.

  • Ability to implement and maintain a self-care program.

  • Strong computer skills, including Microsoft Office Suite and Electronic Medical Records, preferably PS Suite.

  • Second language, relevant to catchment area populations, is an asset.

  • Expectation of participation in community events and outreach activities which may require availability outside of regular hours (e.g. evening and weekends)
