The Diabetes Educator (Registered Kinesiologist) is a key member of a multidisciplinary team of dietitians, chiropodists and nurses providing education and support for adults (18+) living with Type 2 Diabetes, Prediabetes, and other chronic illnesses. Working in collaboration with primary care providers and diabetes specialists, the Diabetes Educator position emphasizes advanced clinical decision making, client teaching and chronic disease management. The Educator serves as a resource and consultant for clients and community partners, integrating population health approaches with direct care.  This role involves interpreting clinical data, using lifestyle management to support diabetes care and staying current with diabetes care standards and technology (i.e. continuous glucose monitoring, insulin pumps etc.). The role carries a caseload and provides individualized counselling and group education on topics such as self-management, nutrition, physical activity, stress management, blood glucose monitoring, and medication management. The role empowers clients to set and achieve personalized health goals for optimal diabetes care. 

Program:

  • West Toronto Diabetes Education Program: A comprehensive diabetes education program, designed to improve the quality of life for clients 18+ living with Type 2 Diabetes or Prediabetes and at risk.

Primary Responsibilities

  • Provide assessment and counselling for clients living with pre-diabetes and Type 2 diabetes, taking into consideration client’s level of knowledge, readiness to change, medical and non-medical factors and socio-demographic background.
  • Develop health improvement plan in partnership with the client by integrating information about health status, clinical practice guidelines, and available community resources.
  • Ensure complete, accurate, and timely documentation of program activities using electronic medical records.
  • Plan, develop and facilitate health education sessions on various topics related to diabetes in both individual and group settings.
  • Participate in the development and improvement of team and organizational procedures, workflows and policies as assigned.
  • Identify and build relationships with various community partners.
  • Other duties as assigned by Manager.

# Qualifications

  •  Bachelor’s degree in Kinesiology and Health Science/ Human Kinetics and in good standing with the College of Kinesiologists of Ontario (COKO) as a R. Kin.

  • Certified Diabetes Educator (CDE) credentials or current enrollment would be an asset.
  • Excellent communication skills, including the ability to communicate information and ideas to support understanding with diverse demographics, newcomers, and individuals with language barriers
  • Strong knowledge of diabetes pathophysiology, related laboratory values, and pharmacotherapy.
  • Experience with facilitating diabetes education and behaviour change counselling; with an Exercise focus.
  • Demonstrated ability to teach and facilitate groups using principles of adult learning.
  • Positive attitude and dependable with strong initiative and the ability to work both independently and in a team-oriented atmosphere.
  • Ability to develop and facilitate group exercise classes in the community. 
  • First Aid and BLS certification required and must be kept current.
  • Valid Driver’s License is an asset.

 Position Summary: 

The Social Worker will work as part of a community-based interdisciplinary team to provide mental health and psychosocial care, therapeutic counselling, case management and follow-up support to clients within the context of a client-centered care model and under the established standards of professional practice. Main Tasks and Responsibilities

Client Care

# Qualifications

 Master’s degree in social work (MSW) preferred.

  • Clinical social work expertise and mental health care with 3 -5 years of experience in a community based, health or social service organization located in a culturally diverse setting.

  • Experience providing clinical counselling and group programming to children, youth and adults.

  • Knowledge and proficiency in social work assessment, trauma-informed care and evidence-based therapeutic counselling models including Cognitive Behaviour Therapy (CBT), Dialectical Behaviour Therapy (DBT), Brief/Solution Focused Therapy and Single Session Therapy (SST).

Key Responsibilities

  • Prior experience (1-2 years) serving on a Board of Directors or senior leadership team

# About the Position

The Alliance is seeking a skilled Privacy Business Analyst to support the delivery, coordination, and continuous improvement of privacy services. This role is ideal for a candidate with strong business analysis capabilities who is comfortable working in a privacy‑focused environment, supporting Privacy Officers and contributing to the effective operation of the Alliance’s privacy program.

The Privacy Business Analyst will be responsible for leading the analysis, assessment, and documentation of business requirements and for acting as the key resource to support Privacy Office initiatives and operational processes. More specifically, this role will support the ongoing operational and client service delivery programs managed by the Privacy Officer. 

# Responsibilities

Privacy Operations Assessment and Requirements Analysis

  • Provide sound business analysis to the Privacy Team, ensuring alignment with Alliance’s privacy strategy, operational needs, and compliance obligations. 
  • Lead the analysis, mapping, and documentation of privacy-related business processes. 
  • Support the Privacy Office Program which offers services to member organizations. 
  • Assess privacy implications on new initiatives at the Alliance or member organizations that require assistance.
  • Review privacy program processes to identify opportunities for improvement. 

Research and Program Improvement

  • Research best practices when it comes to privacy with digital health as new technologies are being utilized by members or those participating in Alliance’s Privacy Office Program.
  • Adapt and enhance privacy tools, templates, and resources to meet the needs of Alliance staff and members, including the development and maintenance of communication channels, guidance materials, training modules, and operational support.

Business Cases Development

  • Prepare business cases, feasibility assessments, cost–benefit analysis, and risk analysis for privacy-related initiatives. 
  • Develop clear options and recommendations to support decision‑making by committees and leadership. 
  • Prepare presentations, briefing materials, and analysis summaries for privacy committees, working groups, and project teams.
  • Represent Alliance’s Privacy Office Program at events (i.e. attending conferences, learning events).

Engagement and Collaboration

  • Work closely with internal teams, member organizations, and external partners to understand privacy compliance needs, gather requirements, and support the implementation of privacy solutions. 
  • Participate in committees, working groups, and project teams to clarify privacy business needs, respond to inquiries, resolve issues, and identify opportunities 
  • Contribute business analysis expertise to planning activities, ensuring privacy considerations are integrated into broader strategic and operational plans.

# Qualifications

  • A postgraduate degree in business analysis, health informatics, or health information management or equivalent work experience. 
  • Minimum 5 years of professional experience in business analysis activities, preferably in the health sector.
  • Demonstrated solid technical capability in business analysis, project coordination, and familiarity with design tools such as Canva, Adobe Illustrator and Photoshop.
  • Ability to prioritize and deliver high‑quality business analysis work in a fast‑paced environment, with sound judgement and accountability. 
  • Apply planning, organization, and critical‑thinking skills to manage multiple priorities and develop practical, evidence‑based solutions. 
  • Ability to build effective working relationships, collaborate well with diverse partners, and provide responsive, service‑oriented support to Privacy Officers and project teams. 
  • Use analytical skills to assess issues, anticipate operational needs, and support informed decision‑making. 
  • Adapt effectively to changing priorities, evolving privacy requirements, and new information or tools. 
  • Communicate complex information clearly, prepare high‑quality reports and business cases, support training and resource development, and maintain confidentiality in all interactions.
  • Proficient with Windows and Microsoft 365 (Outlook, Word, Excel, PowerPoint, SharePoint, Teams, Lists, etc.).
