# Session description

This session builds on developments since the International Foundation for Integrated Care (IFIC) Digital Health Special Interest Group explored the definition of digitally enabled integrated care through its inaugural webinar series in 2019. The ambition remains the same: connected care across hospital, community and primary care.

What has changed is the context — and the challenge of scale. Through international and Ontario examples from HealthPathways and Connected Care/UHN, this session will reflect on how the field has evolved, what has enabled progress, what barriers remain and what these lessons mean for the future of digitally enabled integrated care. Grounded in implementation experience, the discussion will explore adoption, value and how rapidly digital tools are changing what is possible for integrated care.

# Why this matters

Digital tools are now central to many efforts to improve integration across hospital, community and primary care. However, progress is uneven, and scaling what works remains difficult. This session will examine what enables adoption, where implementation becomes challenging and what these lessons mean for the next phase of digitally enabled integrated care.
