This webinar is hosted by the Association of Family Health Teams of Ontario (AFHTO) as part of their Designing Effective Teams virtual learning series.

The AFHTO Designing Effective Teams virtual learning series showcases strategies to maximize the role and scope of interprofessional primary care providers to improve attachment, access, and quality of care, and features guidance, models of care, and innovative practices that can help you design a high-performing team.

# Webinar Description:

As primary care teams expand to meet growing patient attachment goals, there is an opportunity to rethink how the expertise of each team member can be leveraged to support this goal. Primary care pharmacists are an essential - yet often underutilized - resource within primary care teams.

Hosted in partnership with ImPACT, this session will explore how primary care pharmacists are well-positioned to improve access and strengthen the delivery of comprehensive care across a range of primary care settings. Through practical examples and lessons learned from existing models, the session will highlight how intentional role design, interprofessional collaboration, and strategic integration of pharmacists can enhance patient outcomes and build team capacity. Participants will gain insight into approaches that can help organizations maximize the impact of pharmacists within team-based care.

# Learning Objectives
