This document outlnes indicator definitions, calculations, reporting periods, and other technical information hospitals, interprofessional primary care organizations, and long-term care homes will use to complete their 2023/24 Quality Improvement Plans (QIPs). It also inclues the questions organizations will answer in the Narrative section of their QIPs.

Ils peuvent également être utiles à ceux qui participent à la coordination de la prestation de soins virtuels, comme les assistants de bureau de médecin.

This guidance and related tools are intended to help primary care clinicians and those planning and delivering virtual primary care to be purposeful and deliberate in their use of virtual care modalities. It may also be useful to those involved in coordinating the delivery of virtual care, such as medical office assistants.
