Black-Focused Social Prescribing (BFSP) was a 2.5-year pilot initiative that aimed to augment social prescribing practices by developing and applying a culturally affirming model of care, specifically designed to address the unique health inequities faced by ACB communities. The pilot was implemented by The Alliance for Healthier Communities, in collaboration with four Community Health Centres CHCs in Ontario: Black Creek, Somerset West, TAIBU, and Rexdale.

# Purpose

To provide public health authorities and settings such as shelters, group homes, drop-ins, community centres and community clinics with plain language information about reducing exposure to wildfire smoke while considering other indoor air quality goals, including protection from diseases that spread through the air.

For over 50 years, community health centres (CHCs) in Ontario have delivered comprehensive primary health care according to the shared principles described in our Model of Health and Wellbeing (MHWB). Like a neighbourhood home, our member organizations reach beyond their doors to improve the lives of people in their communities. Although it began with CHCs, this approach is now embodied by organizations from multiple primary health care models, who together form the Alliance for Healthier Communities. 

# Executive summary

Public health surveillance involves the collection, analysis, interpretation, and sharing of health-related data to support public health decisions and actions. This essential function enables public health organizations to safeguard the health and safety of people residing in Canada by detecting emerging threats, guiding appropriate responses, and informing prevention strategies.

# About the Project

Challenging Compounding Disadvantage is a community-based research project that explored the compounded effects of the COVID-19 pandemic on immigrants and refugees living with cancer or mental health and addiction disorders.

This brief report provides a definition of community governance, reviews evidence for its positive impact, and describes how it is foundational to the structure and function of Alliance member organizations.
