Alliance for Healthier Communities

For over 50 years, community health centres (CHCs) in Ontario have delivered comprehensive primary health care according to the shared principles described in our Model of Health and Wellbeing (MHWB). Like a neighbourhood home, our member organizations reach beyond their doors to improve the lives of people in their communities. Although it began with CHCs, this approach is now embodied by organizations from multiple primary health care models, who together form the Alliance for Healthier Communities. 

This is a critical moment for our health care system. It presents both a need and an opportunity for transformational change. For over 50 years, the organizations that make up the Alliance for Healthier Communities have been delivering comprehensive primary health care that exemplifies the kind of change we envision for the whole health system, by providing care for people and communities as described in our Model of Health and Wellbeing (MHWB).

The CHC Evaluation Framework Manual lists the data elements to be collected about clients, service encounters, and group programs at community health centres within the Alliance for healthier communities. It also provides brief explanations of the data points and the rationale for collecting them. 

This brief report provides a definition of community governance, reviews evidence for its positive impact, and describes how it is foundational to the structure and function of Alliance member organizations.

Are you interested in measuring the complexity of your organization's clients at a more granular level? 

Staff at London InterCommunity Health Centre (LIHC) have developed a collaborative exercise to measure the complexity of their clinical clients. This allowed them to rebalance their clinicians' rosters, ensuring balanced workloads. In this Lunch 'n' Learn webinar, LIHC staff described the project and provided an in-depth look at their methodology.

This report contains data from the annual Quality Improvement Plans (QIPs) that were submitted to Ontario Health for the 2024-25 fiscal year by the 74 Community Health Centre (CHC) members of the Alliance for healthier communities. It consists of two documents: 

In 2023, the Alliance conducted a pilot study of the EQ-5D tool for capturing patient-reported outcome measures (PROMs) - essentially, a standardized way to measure how clients perceive their health and wellbeing, and track changes in their health and wellbeing over time. This webinar provides an orientation to the EQ-5D, a review of the pilot study and what we learned from  it, and an overview of the supports we will be providing for implementation.
