 We are a dynamic community-based health centre promoting social justice and health equity in the Regent Park neighbourhood and Downtown East area of Toronto. We operate out of several locations and provide a wide variety of services including: primary health care, chiropody, physiotherapy, health promotion, social work, community health, immigrant and refugee services, harm reduction and homeless programs, addiction and recovery services, Pathways to Education, infant and child development programs, parenting programs, and youth employment services.

Position Summary:

The Registered Dietitian - Community Diabetes Educator works with the Diabetes Education team and as part of a multi-disciplinary team to provide care to clients with diabetes. The Registered Dietitian provides individual diabetes education counselling, community outreach and health promotion/education to families and the community.

Specific Duties Include:

• Direct Client Care:
  • Provide one-to-one counselling to diabetes clients to assess client nutritional status by gaining an understanding of food habits or preferences, social-economic and cultural background, psychological factors and medical profile.
  • Develop, implement and evaluate individual nutritional care plans for diabetes clients, considering literacy, language, cultural and psycho-social factors.
  • Draw upon adult education and motivational interviewing skills and techniques during on-to-one and group settings.
  • Plan, deliver and evaluate community nutrition and diabetes education activities for both individuals and groups. Obtain, evaluate and disseminate nutrition education resources in clear language to meet the needs of clients.
  • Ensure continuity of care by maintaining up-to-date client records, and case conferencing with other providers as required.
  • Act as a resource for Health Centre staff, community agencies and groups in the area of medical nutritional therapy for diabetes, pre-diabetes and diabetes prevention.
  • Encounter statistic records for the nutrition programs and service and complete reports to the funder as required 
• Health Promotion and Education
  • Provide diabetes teaching in group settings both in the community and on site 
• Advocate on behalf of clients regarding their food and nutrition needs 
  • Liaise with Regent Park Community Health Centre staff and community agencies in program planning, implementation and evaluation.
  • Provide support to the community and community partners around food and nutrition related initiatives and projects.
• Build and make new partnerships with local community-based organizations to further the reach of these services. 
  • Participate in practice-based research as required.
  • Evaluate program effectiveness and make recommendations for future program direction

# Qualifications

•  University Degree accredited by PDEP and completion of an accredited internship.

• Registered member of the College of Dietitians of Ontario
• A Certified Diabetes Educator (CDE) designation is a strong asset.
• Certification in Intuitive Eating, HEAS, Craving Change and/or Motivational Interviewing is an asset
• Experience working with people from diverse cultures, races, ages, abilities, economic backgrounds, mental health statuses, those with substance use issues, and who experience homelessness. 
• Experience in program planning, delivery, and evaluation
• Knowledgeable regarding community development, harm reduction principles, health promotion and the Social/Structural Determinants of Health 
• Clinical skills in all aspects of assessing and developing a nutrition care plan.
• Knowledgeable of nutrition, diabetes, pre-diabetes and prevention
• Ability to take initiative and work independently 
• Ability to work collaboratively within and across teams
• Organizational and time management skills
• Strong written and oral communication skills, and strong computer skills 
• Understanding of anti-oppression principles and experience with social advocacy
• Knowledge of the Regent Park area and issues affecting the community
• Ability to speak another language is an asset
• Ability to work flexible hours 
• Must be fully vaccinated with COVID-19 for the protection of clients and staff
