# About the position 

 Job Summary

At the heart of our work is an interdisciplinary approach that delivers holistic, community-based care – with a strong focus on individuals who experience barriers to accessing health services.  Our team is deeply committed to inclusivity and works within the social determinants of health framework, emphasizing prevention, health promotion, and client education.

The Community Health Worker (CHW)– Social Determinants of Health – supports health equity in South and Central Etobicoke by delivering health promotion activities focused on healthy aging and the social determinants of health. This role involves outreach, advocacy, community engagement, and program support to ensure inclusive, responsive services for prevailing issues in the community. The candidate will deliver health promotion activities and healthy lifestyle support to high-priority communities through a culturally responsive, harm reduction approach that addresses the social determinants of health and healthy aging.                                                                                                                                                                                                                                                                             Reporting to the Health Promotion & Community Support Manager, the Community Health Worker will play a key role in improving access to health services and wraparound support by facilitating group programs and holistic lifestyle plans, coordinating referrals, leading advocacy and organising outreaches and managing data collection and reporting. The successful candidate will also collaborate with interdisciplinary teams and community partners to design, implement, and evaluate strategies that address the social determinants of health of priority population and promote equitable health outcomes for the community.

Primary Responsibilities

  • Deliver health promotion and education activities.
  • Facilitate community development and outreach.
  • Build partnerships with local organizations and networks.
  • Support communications and program awareness.
  • Assist with data collection, reporting, and program coordination.

# Qualifications

  • •Post-secondary education in a related field.

  • 2+ years’ experience in community health or outreach.
  • Knowledge of health equity and anti-oppressive practices.
  • Strong communication and facilitation skills.
  • Familiarity with Etobicoke communities is an asset.
