CSA Policy Pathways Conference: Advancing Canadian Health Care

This event is hosted by the CSA Group.

Canada’s healthcare system is under significant strain. The lasting impacts of the pandemic, and an aging and growing population, are putting additional pressure on the system. Despite the promise of universal health care, nearly a quarter of Canadians do not have access to a primary care physician, prolonged wait times are increasingly the norm, and some services such as treatment for mental health aren’t covered at all for many.

Canadian health care should be held to a higher standard. Canada needs a holistic, multidisciplinary approach to build a modern system that prioritizes patients’ overall wellbeing.

CSA Group’s inaugural policy conference will bring together thought leaders, practitioners, researchers, and industry experts to advance solutions to the current and looming stresses facing Canada’s healthcare system. CSA’s healthcare conference will address several areas including:

  • Building a robust primary care system
  • Modernizing the Canada Health Act
  • Accelerating healthcare innovation
  • Addressing the social determinants of health
  • Envisioning the future of health care

# Patients Included

ICIC25 is co-designed with patients and caregivers and achieves Patients Included designation. Special consideration is given to papers that demonstrate active people involvement and engagement in either or all of design, implementation and evaluation. Patient and Caregivers attend the conference for free, but we cannot cover accommodation and travel costs. We encourage papers from patient and caregivers, but also suggest that health and care researchers and practitioners should encourage patient and caregiver partners to co-present where appropriate and fund travel and accommodation costs if possible.

The Future is Social Prescribing: Why it's a key to an integrated, accessible, and resilient health system

Presented by the Alliance for Healthier Communities Social Prescribing team.

In 2018, the Alliance for Healthier Communities brought Social Prescribing to Canada. Our version of the model was always a little different: Alliance members worked with their clients and communities to develop programs that were tailored to local assets, values, and needs. This meant embedding system navigation, health promotion, and supports for the social and structural determinants of health. Many of our social prescribing programs address food security, digital equity, and other sources of inequity in addition to social connectedness. Over the last five years, the model has evolved to include special population health focuses such as Black health, mental wellness, and active living for seniors.

In this webinar, we'll celebrate the successes and share what we have learned over the past six years. We will also look to the future of social prescribing and its potential role in advancing health system transformation and integration. For example, we'll talk about the crucial role of Link Workers in connecting to the right supports at the right time and ensuring that health and social care are seamlessly woven together. We'll share our observations about how Social Prescribing can help keep people out of hospital beds and emergency departments, and reduce their need for clinical care. And we'll talk about the incredible potential of culturally tailored Social Prescribing.

Panelists include:

  1. Heather Klein Gebbinck, Executive Director of South Georgian Bay CHC, Wasaga Beach, ON, speaking about the Green Prescribing Pilot project and its successes
  2. Stéphanie Hemmerick, Manager of Community Programs, Seaway Valley CHC, Cornwall ON, speaking about a team-based approach to social prescribing and its successes
  3. Beth Kominek, Health Promoter, Chatham-Kent Community Health Centres, Chatham , ON, speaking about social prescribing successes with Older Adults

Driving Forward: How (and why) to continually advance sociodemographic data collection in your organization.

Over the past few years, we've all learned a lot about collecting sociodemographic data in primary health care. What Alliance teams and coaches have learned can help you accelerate your quality improvement and keep going. Wherever you're at and however far you've come, this is for you. Let's get together to reflect on our progress, learn from each other, and set ourselves up for success on the next leg of the journey.
