LAMP Community Health Centre (LAMP CHC) is partnering with Roots Community Services Inc (RootsCS) and

the Canadian Mental Health Association Peel Dufferin (CMHA Peel Dufferin) to design, develop and implement an integrated health and social services Hub with and for the Black, African and Caribbean (BAC) communities in

Peel Region. The Hub will provide primary health, mental health, addictions and social services programs that will holistically address the physical, mental, social and cultural needs of BAC communities, and seek to eliminate disparities and inequities.  

# About the position 

  • Perform clinical duties utilizing the full scope of chiropodial practice in a tailored approach,  that is, assessing patients for foot health and illness care, treating and preventing disease, dysfunctions, or disorders of the lower limb by therapeutic or palliative means (e.g. physical examinations, advanced wound care, injection of drugs, matrixectomy, patient education, monitoring of chronic illness, treatment of local soft tissue and dermatological disorders of the foot and ankle, and through developing regimes for laser, ultrasound, TENS, mobilisation, and injury-specific exercises, etc.).

  • Respond appropriately to clinical situations/medical emergencies.
  • Listening and choosing how to convey information and ideas concisely in ways that will be clearly understood.
  • Ensuring communication with other health care providers regarding patient care, making referrals as needed for comprehensive care, being involved in outreach and committee work.
  • Document patient visits thoroughly and accurately in the client record daily.
  • Prescribe oral and topical medication for management of pain, inflammation and infection.
  • Prescribe, cast, fabricate and assess appropriate functioning of orthoses and ankle foot orthotic devices and support access to custom made orthopedic shoes.
  • Prescribe, measure, fit and advise regarding compression stockings.
  • Promote healthy behaviours to encourage foot health and on-going increased mobility.
  • Supervise chiropody students.
  • Uses a systematic approach to planning and problem solving both people and thing problems.
  • Adapts to and works effectively with various individuals in a variety of situations or adapt one’s approach to accommodate changes in the situation.  Understanding and appreciating different and opposing perspectives on an issue.
  • Working effectively with patients to achieve common objectives. Taking action to create and sustain highly motivated patients to achieve common objectives.
  • Order, dispense and collect reimbursement for clinical supplies and goods for sale.
  • Ensure equipment is maintained and assist with updating capital inventory.
  • Ensure clinical supplies are available when required.
  • Monitor patient scheduling and waiting list.
  • Bill and collect reimbursement for products sold, maintain financial records, and transfer funds.
  • Know, understand and operate within budget.
  • Carefully monitor expenses on a regular basis.
  • Prepare reimbursement support documents for insurance and ODSP/OW.
  • Other duties as assigned by Manager

# Qualifications

  • Certificate of Competency Current registration/license without limitations with the College of Chiropodists of Ontario.

  • At least three years’ experience, preferably in community-based care.
  • Proficiency in the manufacturing of custom orthotic devices.
  • Excellent interpersonal skills necessary to work effectively across all levels of the organization’s diverse workforce.
  • Excellent communication skills, both verbal and written, with the ability to clearly convey information and ideas.
  • Positive attitude and dependable with strong initiative and the ability to work both independently and in a team-oriented atmosphere.
  • Demonstration of knowledge, understanding and practicing of infection control and prevention.
  • Demonstrated commitment to, and knowledge of, the principles of primary health care, health promotion, and community-based health care delivery.
  • Demonstrated ongoing continuing education in practice and knowledge areas.
  • Cross cultural experience and a second language an asset.
  • Strong MS Word skills as well as basic skills using Outlook, Excel and PowerPoint.

Reporting to the Manager, Primary Health Care, the Diabetes Nurse Educator (DNE) is accountable for the provision of high-quality nursing care to clients as per the standards of the regulated profession, and in keeping with licensed requirements of the College of Nurses of Ontario. The Diabetes Nurse Educator is a key member of a multidisciplinary team of dietitians, chiropodists and nurses providing education and support for adults (18+) living with Type 2 Diabetes, Prediabetes, and other chronic illnesses. Working in collaboration with primary care providers and diabetes specialists, the nurse educator position emphasizes advanced clinical decision making, client teaching and chronic disease management. The DNE serves as a resource and consultant for clients and community partners, integrating population health approaches with direct care. This role involves interpreting clinical data, supporting medication management in collaboration with prescribers, and staying current with diabetes care standards and technology (i.e. continuous glucose monitoring, insulin pumps etc). The role carries a caseload and provides individualized counselling and group education on topics such as self-management, nutrition, physical activity, stress management, blood glucose monitoring, and medication management. The role empowers clients to set and achieve personalized health goals for optimal diabetes care.

Primary Responsibilities

  • Provide assessment and counselling for clients living with pre-diabetes and Type 2 diabetes, taking into consideration client’s level of knowledge, readiness to change, medical and non-medical factors and socio-demographic background.
  • Develop health improvement plan in partnership with the client by integrating information about health status, clinical practice guidelines, and available community resources.
  • Ensure complete, accurate, and timely documentation of program activities using electronic medical 
  • records.
  • Plan, develop and facilitate health education sessions on various topics related to diabetes in both individual and group settings.
  • Participate in the development and improvement of team and organizational procedures, workflows and policies as assigned.
  • Identify and build relationships with various community partners.
  • Maintain accurate, timely EMR documentation; adheres to privacy policies; participates in chart and case reviews and quality initiatives.
  • Collaborate with nurse practitioners, physicians, social workers, dietitians, and others to support a safe, respectful, and equitable care environment.

# Qualifications

  • Bachelor of Science in Nursing (BScN) and in good standing with the College of Nurses of Ontario (CNO) as an RN;

  • Certified Diabetes Educator (CDE) credentials or current enrollment would be an asset.
  • Excellent communication skills, including the ability to communicate information and ideas to support understanding with diverse demographics, newcomers, and individuals with language barriers
  • Strong knowledge of diabetes pathophysiology, related laboratory values, and pharmacotherapy.
  • Experience with insulin titration, oral medication management and use of medical directives is an asset.
  • Demonstrated ability to teach and facilitate groups using principles of adult learning.
  • Positive attitude and dependable with strong initiative and the ability to work both independently and in a team-oriented atmosphere.
  • First Aid and BLS certification required and must be kept current.

Registered Nurse – Intake will support the Unattached Patients Transition Team, a collaborative initiative between LAMP Community Health Centre, and the West Toronto OHT. This role plays a crucial part in improving primary care attachment for individuals who currently lack a regular primary care provider. 

Primary Responsibilities

  • Conduct thorough clinical intake assessments and develop comprehensive cumulative patient profiles (CPPs) for unattached patients.
  • Use advanced clinical judgment and triage expertise to determine appropriate primary care providers or care teams.
  • Document patient goals and complexity factors to support holistic, person-centred care planning.
  • Collaborate closely with CommunitiCare Health, LAMP staff, the Unattached Patients Transition Team, and West Toronto OHT partners to support timely primary care attachment.
  • Provide compassionate, culturally safe, and responsive patient interactions aligned with LAMP’s mission and values.
  • Utilize digital health tools and electronic medical records (e.g., CareTeam, OceanMD, PS Suite).
  • Contribute to staff education, program planning, evaluation and quality improvement initiatives.
  • Work collaboratively within and across interdisciplinary teams and community partners to strengthen coordinated care.
  • Ensures complete, accurate, and timely documentation of program activities and medical records.
  • Monitors for accuracy, quality, and thoroughness through participation in chart reviews.

# Qualifications

Reporting to the Manager, Primary Health Care, the Primary Health Care Nurse Practitioner (NP) is a registered nurse with extended class registration and advanced education. Working in partnership with other clinical and administrative staff in Primary Health Care to provide a full range of health care services to ongoing clients registered to the LAMP Primary Health Care program, the NP places clients at the centre of their care, by providing them with knowledge and resources toward improved overall health and well being.

Clients include the individual, families and the LAMP community. Health care services emphasizes health promotion and disease prevention and the NP practices are evidence-based.

The Nurse Practitioner will ensure that all organizational policies are followed.

Primary Responsibilities

  • Provide comprehensive primary care within the NP–Primary Health Care scope, including assessment, diagnosis, treatment, prescribing/dispensing (per RHPA, Bill 179, and Controlled Drug and Substances Act) and management of health and illness 
  • Consult with physicians as required by CNO NP Standards of Practice or as clinically needed.
  • Deliver reproductive health/obstetrical care within a shared-care model 
  • Provide home visits as needed
  • Maintain accurate, comprehensive electronic health records that meet all required standards of practice and participate in chart audits as well as peer reviews
  • Contribute to the development and review of workflows, policies, and procedures and act as a preceptor for students

Reporting to the Manager, Preventive Care, the Registered Nurse (RN) is accountable for the provision of high-quality nursing care to clients as per the standards of the regulated profession, and in keeping with licensed requirements of the College of Nurses of Ontario. Registered Nurse – Intake will support the Unattached Patients Transition Team, a collaborative initiative between LAMP Community Health Centre, and the West Toronto OHT. This role plays a crucial part in improving primary care attachment for individuals who currently lack a regular primary care provider. 

Primary Responsibilities

  • Conduct thorough clinical intake assessments and develop comprehensive cumulative patient profiles (CPPs) for unattached patients.
  • Use advanced clinical judgment and triage expertise to determine appropriate primary care providers or care teams.
  • Document patient goals and complexity factors to support holistic, person-centred care planning.
  • Collaborate closely with CommunitiCare Health, LAMP staff, the Unattached Patients Transition Team, and West Toronto OHT partners to support timely primary care attachment.
  • Provide compassionate, culturally safe, and responsive patient interactions aligned with LAMP’s mission and values.
  • Utilize digital health tools and electronic medical records (e.g., CareTeam, OceanMD, PS Suite).
  • Contribute to staff education, program planning, evaluation and quality improvement initiatives.
  • Work collaboratively within and across interdisciplinary teams and community partners to strengthen coordinated care.
  • Ensures complete, accurate, and timely documentation of program activities and medical records.
  • Monitors for accuracy, quality, and thoroughness through participation in chart reviews.

Reporting to the Executive Director and serving as a member of the senior management team, the Manager, Finance & Corporate Services provides leadership and oversight for CHCN’s corporate services functions, supporting the organization’s mission, health‑equity model of care, and long‑term sustainability.

This role is less about being the “head accountant” and more about being a people‑centred leader who can guide diverse professional teams, steward public resources responsibly, and ensure strong systems, controls, and relationships across finance, human resources, information technology, data, procurement, and funder accountability.

The Manager plays a key role in organizational decision‑making, operational planning, policy development, and continuous improvement, working closely with the Executive Director, Board, funders, and interdisciplinary leadership team.

Key Responsibilities

Leadership & Team Management

  • Lead, mentor, and support a multidisciplinary team encompassing Finance, Data/Financial Analysis, Information Technology & Cybersecurity, Help Desk, and Human Resources.
  • Foster a collaborative, respectful, and psychologically safe work environment that encourages accountability, learning, and continuous improvement.
  • Oversee workload planning, performance development, and succession considerations across corporate services functions.
  • Model reflective practice, openness to feedback, and inclusive leadership aligned with CHCN’s values and health‑equity commitments.

Financial Stewardship & Accountability

  • Provide oversight of budgeting, forecasting, financial reporting, internal controls, and financial policies, ensuring accuracy, transparency, and compliance.
  • Manage the Centre’s operating and capital budgets; prepare and present financial reports for the Executive Director, Finance Committee, and Board.
  • Liaise with Ontario Health and other funders regarding financial performance, allocations, targets, and variances.
  • Oversee annual audit processes and support audit readiness across departments.

Funding, Grants & Funder Reporting

  • Support the Executive Director and Board in identifying and assessing funding opportunities.
  • Lead the financial components of funding and grant applications, including budgeting, costing, and sustainability analysis.
  • Ensure accurate financial management and reporting of restricted and unrestricted funds in compliance with funder agreements and CRA requirements.
  • Monitor funding performance, risks, and variances, and communicate findings clearly to leadership and the Board.

Information Management, IT & Cybersecurity

  • Provide governance oversight of electronic medical records (EMRs) and all information systems, including hardware, software, and data assets.
  • Ensure appropriate cybersecurity practices, training, risk management, and incident‑response processes are in place.
  • Develop annual and multi‑year IT budgets aligned with organizational priorities and risk tolerance.
  • Oversee vendor relationships and contracts related to information technology services.

Procurement & Contract Management

  • Develop and oversee procurement and contracting processes, approval authorities, and internal controls aligned with public‑sector and funder requirements.
  • Lead competitive procurement activities (RFPs, RFQs, sole‑source justifications), ensuring fairness, transparency, and documentation.
  • Manage vendor performance, contract renewals, and maintain a centralized contract register.
  • Ensure procurement practices support value for money, equity, risk management, and audit readiness.

Human Resources & Workforce Support

  • Support workforce planning, salary benchmarking, and compensation analysis in collaboration with HR.
  • Ensure strong oversight of payroll accuracy, controls, benefits administration, and vendor relationships.
  • Align HR policies and practices with financial controls, funder requirements, and legislative obligations.

Strategic Planning, Data & Decision Support

  • In collaboration with the Data/Financial Analyst and senior leadership, oversee the development of dashboards, reports, and KPIs for internal and external stakeholders.
  • Ensure data integrity, quality, and compliance with funder and organizational reporting requirements.
  • Analyze financial and operational data to support evidence‑based decision‑making, forecasting, and strategic planning across programs.

General Responsibilities

  • Uphold CHCN’s mission, values, and community‑health philosophy.
  • Ensure compliance with privacy, confidentiality, occupational health and safety, and other legislative and accreditation requirements.
  • Identify risks, report concerns, and contribute to a culture of safety and accountability.

Required Qualifications & Experience

  • 3–5 years of progressive management experience in healthcare, community health, public‑sector, or publicly funded human services environments.
  • Demonstrated experience leading and supporting multidisciplinary teams, ideally across multiple functional areas.
  • Experience overseeing budgets, forecasting, financial reporting, audits, and funder accountability.
  • Experience working with senior leadership and Boards, including preparing clear, decision‑focused reports.
  • Strong judgment, problem‑solving skills, and discretion in handling sensitive and confidential matters.

Education

  • A university degree in business, commerce, economics, health administration, or a related field (or equivalent experience).
  • An MBA or CPA designation is considered an asset, but people leadership and system stewardship are equally important.
