Our new health equity-focused, online Social Prescribing training modules are designed for healthcare teams interested in implementing or improving their social prescribing process. These self-directed modules include Getting Buy-in from Interdisciplinary Teams, Co-designing a Process Map, Building an Asset Map, Meeting with Clients, and Using the EMR to Track the Client's Journey, among others. Learn everything from key terms and concepts to practical tips on creating helpful resources tailored to your workplace and working with clients. The modules can be taken individually or as a whole course. For the best learning experience, Google Chrome is the recommended browser for the online Social Prescribing training modules.

ATTENTION all primary care health professionals! We've created an additional tool developed in partnership with the Centre for Effective Practice. This specific 20-30-minute resource, 'Implementing social prescribing in your practice,' is designed for health professionals in primary care, whether working independently or as part of a team, to implement social prescribing in their practice. You can enroll via the link below. Family physicians can now earn up to .5 CERT+/Mainpro+ credits upon completing the program.

Course duration: Approximately three hours. Once you enroll, you will have a six-month period to complete the course; if you need more time, please email the Social Prescribing Team.

Enrollment Instructions:

1. Fill Out the Online Training Form

Click the green button “Enroll Now!” below to complete the form and request access to the online learning platform.

2. Wait for Your Account to Be Created

Once you submit the online training form, the team will create your TalentLMS account. Please allow approximately three business days for the system to send the confirmation email.

3. Check Your Email

You will receive an email from “allianceon noreply@talentlms.com” confirming that your account has been created. This email will prompt you to set your password and will include the link to the platform.

4. Log In and Start the Course

After setting your password, log in to TalentLMS and make sure to select “socialprescribing” as the branch. This will give you access to the Social Prescribing online course page, where you can begin your course.

*Please note that if you have not logged in to your online learning account for 6 months, your account will be automatically deactivated by the system. To reactivate your learning account, please email: socialprescribing@allianceon.org

Certificates: A certificate will be generated upon completion of assigned modules.
