Social Prescribing to Support Primary Care Attachment
This toolkit was produced for use by working groups and support structures of the Frontenac, Lennox, and Addington Ontario Health Team (FLA OHT). However, it may be useful for other mainstream and Indigenous organizations, as well. This document is thus being made publicly available. Please cite appropriately if using it as a reference.
In April 2025, the Alliance for Healthier Communities shared our vision for a Neighbourhood Health Home model of Primary Health Care, which enables regions to attach people to primary health care while addressing health inequities and improving primary care provider experiences. The model offers a path toward connected, equitable, and integrated care for people and communities.

In October 2025, the Alliance for Healthier Communities the officially launched our Interprofessional Primary Care Team (IPCT) Expansion Toolkit. In the first half of this webinar, we provided a brief overview of the Toolkit's contents, followed by a demonstration of how it might help you make the strategic and operational decisions necessary to implement your organization's expansion plans.
