Fax Use Survey

# Share your experiences to help inform fax reduction efforts

In 2022, the Ministry of Health (MOH) – under the Axe the Fax initiative – analyzed the drivers of fax use to identify opportunities to eliminate fax in Ontario’s health system. This initiative included a survey that was sent to key organizations and provider groups to gather preliminary quantitative and qualitative information. 

As part of the Patients Before Paperwork (Pb4P) initiative, Ontario Health (OH) has engaged Deloitte to conduct a refresh of this analysis, identifying opportunities to phase out fax use in Ontario’s health system. While the challenges associated with fax are widely recognized by organizations and provider groups across Ontario’s health sector, the survey refresh will update Ontario’s understanding of current state fax use to begin phasing out fax in Ontario’s health system. To gain a deeper understanding of fax use across Ontario’s health sector, we are looking for your help in completing this survey. 

There are three sets of questions in this survey*:

• Background Questions: To set the context of your care setting, organization, region and role
• Organizational / Practice-Level Questions: To gather insights into organizational / practice-level data
• Personal Experience Questions: To understand your personal experience using fax

*Please note that responses to survey questions should include data on both traditional paper-based fax and electronic fax (e-fax).

There are a total of 29 questions, which will take you 30 to 60 minutes to complete. The survey will close on October 18 at 11:59 p.m. EST. The information provided in this survey will be kept anonymous, but data trends and direct quotes may be used in our findings reports. The link to the survey can be found here: English/French. Some questions may require information to be gathered from others in your organization or practice. To support this, your survey responses will be saved to allow you to leave and come back to finish the survey at any point before the survey close date.
