Brock CHC offers a range of services focusing on the social determinants of health.

If you are a Registered Nurse Diabetes Educator who is determined to make a difference, this is an opportunity for you. In an inter-professional primary care team environment, you will provide diabetes education in both individual and group settings.

Key Responsibilities:

  • Provide diabetes education counselling and related services within the College of Nurses of Ontario’s standards of practice.
  • Utilize the current Diabetes Canada clinical practice guidelines with a client-centered approach.
  • Use individual and group counselling skills to bring about positive client outcomes.
  • Actively participate in an interdisciplinary team environment within the CHC model of care.

Key Qualifications:

  • Graduation with a BScN from a nursing program from a recognized Canadian university.
  • Current registration in good standing with the College of Nurses of Ontario.
  • Minimum two years experience, prior work in diabetes education setting.
  • Certified Diabetes Educator (CDE) an asset. (Must be maintained throughout employment.)
  • Experience documenting in an Electronic Medical Record.

  • Effective time management and organizational skills.

Remuneration: up to $75,536 annually, based on experience. This role will require some evening work and home visits. Competitive compensation package includes HOOPP.
