Quest Community Health Centre is a not-for-profit, interdisciplinary, health care organization with a holistic model of health and wellbeing that focuses on primary care, prevention, health promotion, community capacity building, and services integration, while considering underlying conditions such as poverty, housing, employment issues and food security.
An experienced team builder and manager, you work within the context of the Centre’s Mission, Vision and Strategic Directions to provide leadership in the delivery of integrated and responsive primary health care programs. These are delivered by an interdisciplinary team in a collaborative manner, with an emphasis on health promotion, education and community capacity building within a harm reduction framework.
Working with management team, the position also contributes to the development and implementation of organizational strategies, policies and practices.
Your qualifications include experience in the development and management of programs preferably in a primary health care environment working with interdisciplinary teams. A Masters' Degree, preferably in Community Health, Social Work, Public Health or Nursing or equivalent experience. Knowledge of local community resources and referral processes, and the ability to work with diverse individuals and groups on complex community issues is required. A valid Ontario driver’s license, access to reliable transportation and a Police Reference check are also required.
