South Riverdale Community Health Centre is a non-profit, multi-service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto. The Mobile Relief Worker will operate the field services within the defined catchment area of the COUNTERfit Harm Reduction Program in accordance with established protocols and guidelines. Work will be carried out on weekends, holidays and as needed in providing coverage for the COUNTERfit Mobile Outreach Worker.

This position is expected to work onsite during the COVID-19 pandemic as required.


• Be available on call during weekends, holidays and when outreach worker is absent to deliver harm reduction materials and resources to the homes of service users

• Driver will keep a log of the time calls are received, the time in which they leave to fulfill requests, and the time they arrive to the location

• Return used needles to South Riverdale Community Health Centre utilizing universal precautionary measures

• Follow safety protocols established by the COUNTERfit Harm Reduction Program during fieldwork, including needle distribution and collection

• Provide HIV & Hep C-specific prevention materials and education to People who Use Drugs during shift

• Provide information on and referrals to medical & legal services, treatment programs, income assistance, mental health services and other community resources to people who use drugs

• Track encounters with service users using program database on a daily basis

• Meet with the Program Manager- Harm Reduction and Hep C at least once every two weeks to provide completed Activity Record Forms and to provide feedback on interactions with service users

• Develop and maintain files where appropriate

• Participate in program evaluation and client needs assessment, in conjunction with the Program Manager-Harm Reduction and Hep C

• Regularly provide the Program Manager-Harm reduction and Hep C with time sheets

• Regularly evaluate procedures within own job area

• Compile statistics related to own job area

• Inform the Program Manager-Harm Reduction and Hep C of any controversial incident that has occurred during and off work hours such as episodes with police, threat of violence, etc.


• High School Diploma and/or the equivalent combination of education and work experience

• Some experience in social services or related field preferred, particularly in areas of crisis de-escalation and conflict resolution

• Lived experience

• Valid driver's license, access to own vehicle and insurance (insurance must be kept in the minimum amount of one (1) million dollars

• Knowledge of Harm Reduction Practice

• Knowledge of COUNTERfit Harm Reduction Program

• Demonstrated reliability and dependability

• Basic computer and email skills

• Strong oral and written communication skills

• Ability to read and write legibly

• Strong organizational and interpersonal skills

• Demonstrated respect for diversity, equity, anti-racism and accessibility with the ability to work collaboratively and positively with others including People who Use drugs

• Ability to work effectively with people who use drugs and knowledge of relevant social, health and treatment services available for People who Use Drugs

• Must have knowledge and be comfortable working with marginalized populations, especially people who use illegal drugs

• Ability to work well with others and function as a member of a team

• Must be sex and drug positive

• Preferably a resident of South Riverdale (knowledge of geography streets, etc.)

• Does not use homophobic, trans-phobic, sexist or racist language, or language demeaning to people who use drugs and/or sex workers

• Demonstrates compassion and respect for People who Use Drugs

• Own computer and internet at home an asset

• Proof of triple COVID-19 vaccination status will be required for employment
