Indigenous Housing Services Case Manager
Reports to the Indigenous Housing Services Team Lead
Salary Range: $46,000 - $47,000

Working from the Housing First Approach, the Housing First Case Manager will provide case management supports for Indigenous peoples facing acute homelessness accompanied with complex needs, within the City of Hamilton.

Required Knowledge, Skills and Ability
Case Management & System Navigation

  • Directly and indirectly support a client case load of up to two years, to communicate and liaise with outside community agencies, to address social determinants of health which have impacted an individuals’ leading cause of homelessness
  • Conduct bi-weekly or more frequent home visits to ensure housing sustainability and support regarding personal barriers
  • Develop a care plan with each client based on their wholistic goals and wellness needs.
  • Monitor progress towards goals, reassess and redirect as needed, identify new goals as they arise, celebrate achievements with clients and families.
  • Act as an advocate and support person for clients by accompanying them to housing viewings, community appointments and during case planning meetings
  • Empower individuals to identify their own health and wellness needs, and provide the tools and supports to help them achieve mental, physical, emotional and spiritual wellness.
  • Coordinate access to traditional healing, cultural, or spiritual supports for clients
  • Identify barriers and gaps facing Indigenous people in accessing services and finding resolutions to improve access.
  • Comprehensive knowledge and understanding of the homelessness system and housing first including points of access, referral processes, service eligibility requirements etc.

Partnerships, Education & Promotion

  • Directly build relationships with internal and external programs to raise awareness of the program, and assist with the development of marketing and promotion strategies of Indigenous Housing Services


  • Ensures patient health information is gathered and inputted on a timely basis into the Electronic Medical Record (EMR).

STATEMENT OF QUALIFICATIONS: In addition to requirements for the position, the ideal candidate would also possess some of the following qualifications:

  • Completion of a post-secondary diploma or degree in a social work, addictions or health care related field preferred.
  • 1+ years experience working with individuals with complex needs
  • Knowledge of case management/housing systems, models and principles.
  • Experience working with and extensive knowledge of Indigenous Peoples language, culture, customs and traditions
