South Riverdale Community Health Centre is a non-profit, multi-service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto. Our mission is to improve the lives of people that face barriers to physical, mental, spiritual and social well-being. We do so by meaningfully engaging our clients and communities, ensuring equitable access to primary health care and delivering quality care through a range of evidence informed programs, services and approaches. The Health Promoter – Choose Health coordinates and implements the health education/ promotion/social prescribing programs for the community and facilitates the integration of a health promotion focus in CHC activities as a member of a multi-disciplinary team. This position works collaboratively with community groups and agencies using a community development model. This position is further expected to be available at hours that are suitable for community members and volunteers. This may mean early morning and evening hours as well as weekend time.
This position is expected to work onsite during the COVID-19 pandemic as required.
- Leads the development, planning and implementation of Self-Management Programs
- Acts as a facilitator as required and function as Master trainer for Chronic Disease Self-Management Program and Chronic Pain Self-Management Program and other programs as required
- Anchor partner liaison, supervision of peers, partner recruitment and outreach, and peer facilitator mentor/support
- Oversee and recruit program facilitators
- Explore service partnerships in collaboration with management team
- Network with internal and community partners to ensure referral to available self-management patient education programs and healthcare provider trainings
- Develops and implements outreach strategies to connect with marginalized community members living with long term health conditions in order to refer self-management programs
- Implements service provision in an appropriate and culturally competent manner to a culturally and socio-economically diverse range of communities
- Work to develop a communications/marketing strategy to promote awareness of the SMP and related activities among individuals with or at risk of chronic disease including diabetes and their health care providers
- Lead program team in production of program material, brochures, and flyers
- Routinely maintain and update www.selfmanagementtc.ca to promote upcoming chronic disease programs/events
- Identify, recruit, and organize all self-management training.
- Supervise, support, and mentor Health Care Provider facilitators and Peers with lived experience
- Develop and implement strategies to maintain an active and engaged trainer/volunteer peer leader/mentor community
- Facilitate ongoing peer facilitator training and coaching sessions
- Recruit participants and support partner organizations with participant recruitment
- Represent the program with strong knowledge of overall program especially when developing new partnerships in conjunction with management team
- Coordinate program evaluation activities including audits completed by master trainer with support of health promoter or health promoter in capacity as master trainer
- Collect, summarize and report data aligned with the initiative’s deliverables, including evaluation data
- Provide support to participants and facilitators including form completion, systems navigation, advocacy and social prescribing and referrals to address needs that are not therapeutic or medical
- Leads Choose Health team in collaborating with other teams across the organization to identify and provide self-management health education to target groups
- Establish community development initiatives in alignment with SRCHC’s vision
- Participate as a team member in all team functions program planning, team meetings, facilitator meetings, and partner meetings
- Support delivery of virtual and in person workshops as needed
- Analyse data and prepare program reports as per program directives and funder/organizational requirements
- Write funding proposals to secure additional resources to support the Self-Management Programs
- Oversee processing of program invoices for manager signature
- Oversee program participant referral and booking system and establish protocols
- Oversee and maintain paper and electronic file management systems following established protocols
- Oversee and ensure data entry and program database to maintain accuracy and integrity of client and partner information
- Maintain and develop professional competence and learning
- Supports workshop licensing, logistics and delivery including referral processes and strategies to ensure a successful self-management program
- Identifies, with community members, health related education needs and plans, develops, implements and evaluates health education programming – using various formats such as community events, group programming, use of media and presentations to large groups
- Disseminates accessible health education materials, developed with community members
- Uses a health promotion approach in the Choose Health program by raising awareness and advocating for appropriate programs and service to support community members, healthcare providers, and caregivers in their journey to respectively strengthen self-management skills, support rapport building and learn skills to strengthen self-care
- Participate in infection control measures throughout the facility
- Assist with front door screening of clients
- Other duties as required
- Post-graduate degree from a recognized university in public health, social science, adult education, or a related discipline, or a combination of undergraduate degree and significant experience
- 3-5 years health promotion/education experience in a community-based organization
- Minimum 2 years’ experience working with marginalized populations
- Excellent group facilitation skills, experience and knowledge of health promotion and education, community organizing techniques and program design, implementation and evaluation
- Knowledge and experience with social prescription programs
- Experience in community outreach
- Experience in community education and engagement
- Thorough knowledge of community development approaches
- Knowledge of community resources
- Demonstrated commitment to and knowledge of community-based health care and resources
- Proficiency in peer education strategies
- Proficiency in the use of computer technology and various software applications
- Strong oral and written communication skills
- Beginner level skills or a willingness to learn communications platforms including Wordpress, MailChimp, etc.
- Excellent interpersonal skills
- Demonstrated skills in partnership work
- Demonstrated respect for diversity, equity and accessibility with the ability to work collaboratively and positively with others
- Demonstrated ability to work effectively in a multi-disciplinary team environment
- Excellent coordination and organization skills
- Some project management experience
- Experience in working with our priority populations, including people living in poverty, those with diabetes and chronic disease, mental health and/or substance use issues as well as newcomers/immigrants
- Knowledge/interest in chronic disease self-management from various perspectives including individuals living with chronic condition(s), healthcare providers and caregivers
- Police clearance documentation will be required for employment
