Community Health Nurse RN - Primary Health Care
Contract position,- 35 hours per week until March 31st 2021
$64,080 to $75,915 per annum
(salary commensurate on experience & education)

Operating out of six locations, Unison offers core services that include primary health care, counselling, health promotion, Early Years programs, legal services, harm reduction programs, housing assistance, adult protective services, Pathways to Education™ and a Diabetes Education Program. Unison’s mission is working together to deliver accessible and high quality health and community services that are integrated, respond to needs, build on strengths and inspire change.

We are seeking a candidate for the position of Community Health Nurse to join Unison’s Primary Health Care team based at our Keele-Rogers site. The ideal candidate for this position will have a strong background in clinical primary care,Covid testing, experience in health teaching, advocacy and health promotion.

Job-Specific Responsibilities:

  • Provision of individual primary care including wellness assessment across the lifespan, immunization, chronic disease monitoring, triage, counseling, advocacy and care of minor episodic illnesses
  • Health teaching to individuals
  • Working collaboratively within a multidisciplinary team
  • Provides clinical support and/or backup at the other sites when required
  • Participation in team meetingss and centre-wide initiatives
  • Performance of administrative tasks
  • Participates in Unison’s on-site Covid-19 testing, and external locations via Mobile Clinic

Job-Specific Qualifications:

  • Licensed as a Registered Nurse with the College of Nurses of Ontario with no restrictions to practice
  • Baccalaureate degree or equivalent experience
  • Strong understanding of and experience working with Covid testing and clinical assessment skills
  • Strong documentation and communication skills
  • Excellent English communication skills, both written and verbal, with individuals and groups; knowledge of a second (or more) language and culture is an asset
  • Sensitivity to the impact of social health determinants on a diverse, urban, low-income community
  • Sensitivity to and knowledge of the needs of a multicultural, multiracial community
  • Experience serving Unison’s priority populations (people with mental health and addictions, seniors with complex needs, LGBTTQ2+, and youth facing barriers)
  • Current RNAO membership with malpractice coverage is an asset
  • Ability to work flexible hours.
